The Chronic Disease Management/Diabetes Patient Care Coordinator/Case Manager provides services for the Crow Service Unit (Crow/Northern Cheyenne Hospital, Lodge Grass Clinic, and Pryor Clinic) in the Crow and Northern Cheyenne Communities. This position is located under the Outpatient Education Center working with the diabetes team members to meet the Diabetes Center of Excellence Strategic Goal and support the Special Diabetes Program for Indians: Good Medicine - Baa Laa Wii Lalaa program.

Duties

The primary role of this position is to support patient care coordination and case management for patients who are referred to the Outpatient Education Center for chronic disease management and/or identified as having 2 or more diagnosed chronic disease states. Direct Patient Care: Identify individual patient's needs and assist the patient and the patient's family in establishing treatment goals by counseling and recommending appropriate treatment modalities to satisfy the objectives of therapy. Community Collaboration: Participate in Diabetes Program with multidisciplinary team members to assess and provide culturally relevant care for patients living with Diabetes. Staff Development: Provide technical assistance and guidance in planning, implementation and evaluation of diabetes continuing education programs for all nursing personnel at the Service unit; assist in coordinating professional education training programs on a continuous basis to IHS diabetes team members at the Hospital and Health Centers through participation in planning and implementing IHS Diabetes Care Model. Program review/Diabetes Audit: Participate in the analysis and interpretation of collected data by relating results of knowledge of Diabetes and health care delivery systems which might cause or explain any variations.

MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level (GS-9) in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Plan and carry out assignments, assess diabetes education and status of Diabetes Standards of Care, discuss treatment plan with physicians. Coordinate care with other departments. Work with patients in the specialty of diabetes treatment and prevention in a community or clinical setting. In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
