This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements of Whole Health System.

Duties

Duties may include: Facilitates and empower patients to develop and achieve self-determined goals related to health and wellness. Develops self-management strategies for making sustainable, healthy lifestyle behavior changes and more effectively managing chronic disease. Assists patients with weight management, smoking cessation, and addressing problematic areas in the eight components of proactive health and wellbeing. Contacts eligible participants for enrollment and explain the health coaching process. Provides follow-up care through telephone clinics or secure messaging between appointments. Reviews current health behaviors and develops customized treatment plans with patients. Utilizes motivational interviewing and health coaching techniques and processes, learned through pre-requisite health coach training, to assist members in developing a realistic plan to accomplish their goals. Provides educational opportunities for staff-including lunch and learns, grand rounds, and presentations at staff meetings-in order to promote health coaching and encourage referrals to these services. Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues. Ability to retrieve, track and trend information needed for reporting related to health coaching. Assists with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews. Develops and analyzes relevant data that can be used to prepare analytical and interpretive reports and guides. Participates in leadership roles on local, state and/or national professional organizations specific to health coaching. Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues. Conducts outreach activities to inform patients and family members of available programs Utilize a variety of computer systems to create reports, lists, and disseminates the information. Prioritizes, plans, and carries out all assignments independently. Responsible for day-to-day health coaching operations and maintenance within the service. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. Work Schedule: Monday through Friday; 8:00am-4:30pm; Subject to change based on the needs of the facility. Telework: Yes; Ad-hoc telework only. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 04/24/2024. Individual Occupational Requirements: Applicants who meet the Individual Occupational Requirements described below are fully qualified for the specified entry grade and must also meet the specialized experience and/or education requirements. To qualify you must possess one of the following: Basic Requirements: Degree: behavioral or social science; or related disciplines appropriate to the position. (TRANSCRIPT REQUIRED) OR, Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. (TRANSCRIPT REQUIRED) OR, Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. In addition to meeting the above Individual Occupational Requirement (IOR), applicants must also meet the experience and/or directly related education relevant to this position. AND Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-05. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. AND You may qualify based on your experience and/or education as described below: GS-07: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Review supporting material to determine compliance with regulations and policy requirements; Plan and implement program(s) within a medical center setting; Establish creative engagements to Veterans, employees, and community partners in a newly developed program; Present and obtain data in a wide variety of formats, such as public speaking to large groups both in-person and using telehealth technology; documenting Personal Health Inventories/assessments, SMART (Specific, Measurable, Attain, Realistic, and Timed) Goals, & Personal Health Plans in computerized patient records; Develop training materials for educational needs through qualitative and quantitative methods. OR, Education:1 year of graduate-level education (transcripts required). OR, Superior Academic Achievement (SAA) (transcripts required).S.A.A. is based on (1) class standing, (2) grade-point average, or (3) honor society membership. Class standing -- Applicants must be in the upper third of the graduating class in the college, university, or major subdivision, such as the College of Liberal Arts or the School of Business Administration, based on completed courses. Grade-point average (G.P.A.) -- Applicants must have a grade-point average of: 3.0 or higher out of a possible 4.0 ("B" or better) as recorded on their official transcript, or as computed based on 4 years of education, or as computed based on courses completed during the final 2 years of the curriculum; or 3.5 or higher out of a possible 4.0 ("B+" or better) based on the average of the required courses completed in the major field or the required courses in the major field completed during the final 2 years of the curriculum. OR, Combination of Specialized Experience and Education: Combination of specialized experience as described above, and graduate education or superior academic achievement as described above which totals at least one year. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent 1 year of full-time study. If that number cannot be obtained from the school, 18 semester hours should be considered an academic year of graduate study. (transcript required). GS 09: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Analyzes current research literature on health and wellness, including behaviors that promote a healthy lifestyle and well-being. Assembles health and wellness coaching techniques to promote positive behavior change using one-on-one, couple, or group modalities. Corresponds effective interpersonal communication techniques to develop and maintain strong interpersonal relationships. Directs the client in developing a comprehensive, personalized health plan based on a patient's action-oriented goals. Identify resources internal and external to the organization that are necessary for achieving patient overall health and well-being goals. Interprets information technology (IT) equipment and various/emerging technologies to carry out work responsibilities. Transcribes patient information using appropriate systems. OR, Education: 2 years of progressively higher level graduate education leading to a master's degree or master's or equivalent graduate degree (transcript required). OR, Combination of Specialized Experience and Education: Combination of specialized experience as described above, and graduate education. When combining education with experience, one year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent 1 year of full-time study. If that number cannot be obtained from the school, 18 semester hours should be considered an academic year of graduate study. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended. (transcript required).
