The purpose of this position is to provide a social worker to facilitate social services for the Peach Springs Clinic, Peach Springs, AZ and Havasupai Health Station, Supai, AZ. The incumbent reports to the Supervisory Social Worker.
Duties
Plans, develops, implements, and modifies the social work programs.
Evaluates the existing services and trends evidenced by workload demand sources of referral, types of patients/cases, community expectation and availability of other resources.
Serves as the patient advocate to address and initiate the resolution of patient complaints to ensure effective continuity of care.
Participates and attends all case management meetings and case staffing to assist in the development of patient treatment plans.

Qualifications
To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
Individual Occupational Requirements:
BASIC REQUIREMENT(S):
Degree: Social work Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council of Social Work Education.
In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below-
MINIMUM QUALIFICATIONS:
Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-11 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Assisting patients and their families in utilizing available community, state, and Federal resources for the purpose of improving their social, physical, psychological health; reporting on patient who are involuntarily committed; serving as a liaison between patient, physician/clinic, or other members of the team including community, state, and Federal agencies; completing monthly statistical and narrative reports on all services provided; provide individual and family therapy to out-patients.
Selective Placement Factor: This position requires a current, unrestricted U.S. State license as a Licensed Independent Social Worker (LISW), a Licensed Clinical Social Worker (LCSW), or a Certified Independent Social Worker (CISW). Please provide a copy of your license with your application.
