The Advanced Practice Nurse (APN) functions directly to support the activities of the St. Cloud VA Health Care System REC ICC. The APN is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. The REC ICC foundational elements will serve to support delivery of high quality, timely care to Veterans receiving services.
Duties
The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. The CLC Advanced Practice Registered Nurse (APRN) functions directly to support the activities of the St. Cloud VA Health Care System REC ICC.
Responsibilities but not limited:
Responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care.
The REC ICC foundational elements will serve to support delivery of high quality, timely care to Veterans receiving services.
The lead of the integrated care team established by the REC ICC.
Responsible for providing clinical leadership and expertise on REC ICC processes and regulations.
Serve as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community.
Possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence-based standards of care, compliance guidelines and local policy related to Delegation of Authority as established by the facility Chief of Staff.
Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
The APRN's functional role includes following the standardized care coordination model, incorporating case management principles if applicable, knowledge of processes related to referral and authorization and secure medical documentation exchange.
Ability to identify Veterans requiring disease management and the process to obtain these resources internal or external to the VA if applicable.
Focus on providing care that is unique to Veteran needs and continues to monitor, track and intervene, if necessary, in the episode of care across the continuum.
Demonstrate effective communication skills to collaborate and co-manage the Veteran's care with the multidisciplinary team in a cost-effective manner.
Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process that includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.

Physical Requirements: Heavy lifting (45# or more), moderate carrying (up to 45#), reaching above shoulders, use of both hands and legs, use of fingers, prolonged walking and standing, intermittent crawling, kneeling, and bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far binocular correctable to 20/40, vision, hearing (aide permitted), mental and emotional stability.
