The James J. Peters VA Medical center is currently recruiting for ONE Social Worker for the HUD-VASH Program is located within Community Support Services co- located under the Mental Health Care Line and Social Work Service Line. The HUD VASH Social Worker (Program Coordinator) serves an administrative and clinical liaison to the contract provider{s). The HUD VASH Social Worker (Program Coordinator) also provides leadership on a team, as part of a workgroup or in implementing an initiative.

Duties

Some duties include but are not limited to: Administrative Duties Serves as the medical center's primary point of contact for the National HUDVASH Program, Office of Mental Health Services, and VACO. Serves as the Contracting Officer Technical Representative (COTR) for the medical center HUD-VASH contracts and other contracts as assigned. Maintains COTR certification through ongoing training and continuing education as mandated or requested by Network Acquisitions Office/ Contracting Department. Develops a working relationship with the Contracting Officer and community-based contract provider; ensures efficient and effective contract implementation at the medical center and in the community. Conducts monthly, quarterly, and annual performance reviews of contract provider services and delivery in accordance with the contract Statement of Work and the Quality Assurance Surveillance Plan. Efficiently and effectively resolves performance issues with the contract provider whenever possible through problem solving strategies and reasonable accountability methods. Actively communicates unresolved performance issues to the Contracting Officer in a timely way. Reviews monthly invoices and facilitates payments to contractors as directed by Contracting and Fiscal requirements. Resolves errors in billing efficiently. Develops a working relationship with the designated HUD point of contact and facilitates problem solving with the contract provider in the housing voucher application and award process for Veterans as needed. Clinical Duties Ability to provide training, orientation, consultation, and guidance within clinical specialization of practice. Providing leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs. Clinical program development and accountability for clinical program effectiveness and modification of service patterns. Incorporating complex multiple causation in differential diagnoses and treatments. Providing outreach services to engage homeless Veterans, to include building collaborations with both internal and external stakeholders to ensure service delivery methods and service patterns match the identified patient need. Utilizing Housing First and other evidence-based practice models to promote Veteran engagement and self-efficacy. Screens all Veterans referred to the HUD-VASH contracted case management program via admission screening team meetings. Coordinates the meetings as well as maintain data logs with outcomes of the team meeting. Conducts monthly chart audits of cases assigned to contract provider to ensure compliance with contractual obligations as well as quality of care provided to veterans served. Maintains accurate records of admission and discharge dates of Veterans utilizing HUD-VASH vouchers and meets with the Community Support Services (CSS) Manager regularly to review progress and performance. Takes responsibility for improving professional skills, participating in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs as needed or assigned. Performs accountability functions: 1) documents all contacts with patients/families in the medical charts; 2) completes statistical, administrative, staffing data and other reports in accordance with VA Medical Center and respective professional discipline service policies. Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Veterans utilizing HUD-VASH vouchers. Assisting the Veteran with housing placement, beginning with the process of obtaining a HUD-VASH voucher from the Public Housing Authority (PHA) through the tease up process. Other duties as assigned, as related to the needs of the program. Work Schedule: M-F 8am - 4:30pm Pay: Competitive salary and regular salary increases Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: US Citizen Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Physical Requirements: Physical Requirements. See VA Directive and Handbook 5019, Employee Occupational Health Services. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determinations: GS-12 Social Worker- Program Coordinator Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] Preferred Experience: Previous COR experience References: VA HANDBOOK 5005/120 PART II APPENDIX G39 The full performance level of this vacancy is 12.
