This position is a Nurse (Care Manager) position for the Outpatient Department at the Watonga Indian Health Center in Watonga, OK. This position assists in the development of the plan of care, ensuring that the plan is implemented on a timely basis, and identifying patient needs, promoting an interdependent collaboration with the patient, primary care provider, family, and other members of the health care team. The incumbent reports to the PHN Supervisor of the Clinton Service Unit.

Duties

Coordinates clinic visits for assigned patients in order to optimize care. Conducts pre-visit planning and communicates patient needs and recommends treatment to care team members. Serves as the primary contact/clinical liaison between referral hospitals, treatment facilities, and patients. Implements and coordinates specific caseload strategic interventions to move patients through the continuum of care. Provides direct nursing care and documents care in electronic health record.

INDIVIDUAL OCCUPATIONAL REQUIREMENT(S): Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. BASIC REQUIREMENT(S): In order to be considered for this position, you will need to meet the following requirements: Education: A graduate or higher-level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. (Include transcripts or list in resume) - AND - Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. (Include a copy of your license, online printout, or list in resume with expiration date) In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: GS-10: You must meet one of the following requirements to meet the GS-10 grade level: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the GS-09 grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: performing specialized nursing assignments such as continuity of care for pre-visit planning, visit coordination and post visit follow-up care; OR Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
