Duties
The Community Living Center (CLC) Nurse Practitioner (NP) is responsible for providing competent, evidence-based care to assigned residents. The NP partners with the resident, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum. This nurse practitioner has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
The Community Living Center (CLC) is a 38 private-bed inpatient unit located within the VA Medical Center. This position is Monday-Friday 0730-1600, no weekends, no holidays or call. The CLC takes care of a variety of veterans with complex care needs. These include but are not limited to wound care, IV antibiotics, rehab, dialysis, mental health, chemo/radiation, hospice/palliative care, respite care, dementia, and skilled nursing. Length of stay averages around 28 days for veteran on the CLC.
The Community Living Center is supported by many ancillary services and specialty care providers. Interdisciplinary team includes but is not limited to nursing staff, social work, full time psychologists, chaplain, dietary, and rehab staff. Regular care conferences and rounds occur on the unit to keep all team members, family and the veteran informed on the plan of care. The NP is responsible for admissions, history & physicals, discharge coordination and summaries and intermittent rounds addressing changes in clinical status.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
