DUTY LOCATION: G1, Niantic, CT AREA(S) OF CONSIDERATION: Area I: All On Board Federal Employees (T5), Tenure 0,1,2,3 within the CTNG with U.S. citizenship. Area II: All On Board Federal Dual-Status Military Technicians (T32), Tenure 0,1,2,3 within the CTNG with U.S. citizenship. Area III: All current and former members of the Connecticut National Guard with U.S. citizenship. Area IV: All current Federal Employees from any agency. Area V: All U.S. Citizens.

Duties

THIS IS A NATIONAL GUARD TITLE 5 EXCEPTED SERVICE POSITION This is an indefinite position that may convert to permanent without further competition. This National Guard position is for a NURSING (CASE MANAGER)(Title 5), Position Description Number T5637P01 and is part of the CT G1, National Guard. JOB SUMMARY: The primary purpose of the position is to facilitate medical readiness and medical retention for National Guard military members. Using a collaborative team approach, assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet complex health needs of Soldiers in order to promote health, wellness, and deployability. Addresses patterns or problems at the patient, provider, and/or systems level to promote smooth transition of Soldiers through the Military health care experience. Seeks resolution for identified problems, through individual or collaborative efforts to assure appropriate utilization and optimization of resources. Position provides non-clinical case management to provide pre-mobilization and post-mobilization Individual Medical Readiness (IMR) support to include identification and tracking of Service Members (SM) who are not Fully Medically Ready (FMR) either for dental or medical concerns. Cases will be obtained from Annual Periodic Health Assessments (PHA) events, Home Station (HS) or Mobilization Stations (MOBSTA) SM Readiness Processing (SRP) events or identified for those SM determined non- medically or non- dentally ready. Case Managers shall assist SMs to become Fully Medically Ready (FMR) by assisting with tracking medical and dental appointments, ensuring medical compliancy by maintaining communications as required with the SM they are managing. The major duties include, but are not limited to: Interpret and translate SM limitations from non-military providers. Provides advice, guidance, and health teaching to SM and Chain of Command, in individual or group settings designed to assist the SM in understanding, accepting and following medical recommendations. Coordinates and implements health prevention and education programs related to fitness for duty, medical readiness, and deployability. Assumes responsibility and accountability for case management of all Soldiers in case load. Assists in ongoing patientcare coordination until planned outcomes are achieved.

Requirements

  • Must be HIPPA trained and maintain certification.
  • May occasionally be required to work other than normal duty hours; overtime or compensatory time maybe required.
  • Designated as Essential Personnel and may be subject to duty in preparation for, or in response to, a state emergency or disaster declaration. This designation will not exceed 14 calendar days per year unless otherwise approved in advance by the TAG.
  • Must be able to establish effective professional working relationships with Chain of Command, coworkers, and customers, contributing to a cooperative working environment and successful accomplishment of the mission.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must maintain current certification in Occupational Health from the American Board of Occupational Health Nurses.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must maintain a current license as a Registered Nurse.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must maintain a current certification in cardio-pulmonary resuscitation.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must maintain an N-95 or fitted gas mask respirator if indicated by site hazards.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must be medically fit to wear respiratory protection and protective clothing if indicated.
  • As a basis of continued Title 5 Excepted Service employment/appointment to the GS-0610 position, the employee must complete annual assessments for his or her immunization status and ability to wear respirator if indicated.
  • This position requires a bachelors with an RN or an RN with a certificate and 2 years of experience, or an LPN with minimum of 2 years of experience, and active state licensure or transferable license from other states.

Qualifications

MANDATORY EDUCATION & LICENSURE REQUIREMENTS: Applicants must provide official transcripts from accredited universities or colleges. To qualify for these positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Must have an Undergraduate Degree in Health Administration, Health Care Administration (or equivalent), Health Care Management, Health Management and Policy, Health Services Administration (or equivalent), Hospital Administration (or equivalent), or related fields such as, Primary Care Management, and Bachelors of Science in Nursing (BSN/RN), Biology, or microbiology. Bachelor's degree in nursing (BSN/RN) or Biology. Other completed degrees are acceptable in Public Health, Entomology, Microbiology, Zoology, Biomedical Engineering, Clinical Engineering and Health Management/Health Systems Engineering, Operations Research, or other closely related degree fields. -OR- Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Certification as an Occupational Health Nurse or a Graduate Degree in Public Health (MPH/MSPH) with a bachelor's degree in a biological science. Other educational options are a Master's in Health Administration, Health Care Administration, Health Care Management, Health Management and Policy, Health Services Administration (or equivalent), or Hospital Administration. A Master's degree or equivalent in public health, and one year of graduate medical education completed in a clinical specialty at a hospital is strongly recommended. GENERAL EXPERIENCE: All on-board NG Title 5 Excepted Service GS-0610 Occupational Health Nurse employees must be certified at the next higher grade-level before considered as eligible for merit promotions to other Title 5 Excepted Service GS-0610 Nurse positions; and must have a minimum of one year (12 months) on-the-job experience in the grade immediately below the next higher level. However, a new hire applicant, to include an on-board Title 32 or a Title 5 Competitive Service employee, who is employed in a different series' position, must have at least two years of specialized experiences that are commensurate or equivalent to the GS-0610 Nurse Program business, with demonstrated understanding of the principles and concepts defined by a practical knowledge of health services activities such as, administration, managed care, human resource management, logistics management, patient administration, budgetary and fiscal management, medical manpower, medical facility management, biometrics, medical recruiting, or aeromedical evacuation. SPECIALIZED EXPERIENCE: Your resume and any other supporting documentation you wish to provide must clearly substantiate you currently possess 6 months of specialized experience in the following: Experience demonstrating knowledge of the basic principles and practices related to the management of occupational health care delivery systems. Experience planning for and the execution of state occupational health and industrial hygiene programs. Experience in one of the medical service core functions, which are prescribed as: Health facilities, health plan management, medical resources, health care information, management and information technology, medical logistics, and medical readiness. Experience in written and oral communication. Experience, education, and/or training which provided working knowledge of the principles and practices related to the management of occupational health care delivery systems. Experience planning, developing, analyzing, and briefing the occupational health nursing services provided to National Guard personnel throughout the state. Experience developing and maintaining a comprehensive Occupational Health Program that involves the occupational health nursing services and program requirements. Experience determining negative or positive occupational impacts on National Guard military and civilian personnel and public health. Experience formulating, analyzing, revising, and implementing policies, procedures and best practices in order to accomplish program goals and objectives in accordance with Federal, State and local laws. Experience demonstrating knowledge in governing laws, such as Occupational Safety Health Administration (OSHA) Act of 1970; Federal Employees Compensation Act - FECA; Department of Labor; Department of Transportation; and, the Nuclear Regulatory Commission statutes; and, all applicable DoD Occupational Health Program guidance. Experience providing expert advice to the Adjutant General to protect National Guard personnel against adverse effects of health hazards. Experience determining if personnel are physically, mentally and psychologically suited for work in field operations and the industrial workplace. Experience providing medical care and rehabilitation of occupational illness/injury. Experience identifying physical deficiency; skilled in preventing decreased combat readiness caused by occupational illness/injury to military personnel; and able to provide corporate review, analysis and management of worker compensation and line-of-duty costs. Experience consulting on occupational health issues and experiences included work with the Civil Support Team, Enhanced Force Readiness Package commanders and the Radiation Safety Program. Experience demonstrating the ability to support work and oversight to military and/or civilian personnel units exposed, or potentially exposed to chemical, biological, or radiological hazards in the work environment. Experience evaluating and administrating comprehensive occupational health programs, services, and capabilities for military and civilian personnel in the work environment to include field operations, garrison, industrial and administrative sites. Experience demonstrating skill and knowledge on how to apply information management resources such as, the DoD Defense Occupational Environmental Health Readiness System (DOEHRS); Medical Operational Data System (MODS); also, the DoD Occupational Health Program Resource Models; Occupational Health Information System (OHMIS). Experience demonstrating a range of program elements such as, epidemiologic investigations of occupational illness and injury; industrial hygiene surveys; prevention, mitigation and treatment of occupational illness and injury; emergency treatment of non-occupational illness and injury; hearing conservation and readiness; occupational vision conservation and readiness; radiation exposure and medical surveillance; job-related immunization; illness absence monitoring; occupational health related work-site and industrial inspections; and health hazard education. Experience managing job-related hazards, such as chemical, biological, radiological, nuclear, and explosive hazards. Experience performing job-related occupational health screenings and medical surveillance examinations to included pre-deployment and post-deployment follow-up, pre-placement, baseline, job transfer, periodic, retirement, termination, fitness-for-duty, and health maintenance exams. Experience demonstrating knowledge of how to wear personal protective equipment; skill in using an automated external defibrillator and skill in performing first-aid and critical care education IAW Service specific manuals, instructions, and regulations.
