The Community-Based Outpatient Clinic Associate Nurse Executive (ANE) is a high-level nursing leader responsible and accountable for nursing care provided within primary care at the Community-Based Outpatient Clinics (CBOCs) affiliated with Columbia VA Health Care System.

Duties

The CBOC ANE is a key member of the multidisciplinary leadership team which is responsible for promoting and enhancing the Patient Aligned Care Team (PACT) model of care. Responsibilities include but are not limited to: Implementing, planning, organizing, directing, coordinating, reviewing, evaluating, and improving the delivery of nursing services within Primary Care utilizing the Patient Aligned Care Team (PACT) model of care. This includes additional services within the CBOCs such as but not limited to Women's Health, SCI, Whole Health, Toxic Exposure Screening. Provides expertise in practice management to include extensive team-based PACT guidance to all Community-Based Outpatient Clinics focusing on improving access, promoting PACT roles and responsibilities, delivering quality care, and enhancing the patient experience. Responsible for overseeing timely completion of today's work today with closing encounters, progress note completion, view alert management, addressing secure messages. Investigates and actions timely completion of patient complaints (PATS R) and safety concerns (JPSR). Works collaboratively with multiple disciplines to plan and implement specialty services within the CBOCs to expand access to care. Extensive involvement in the growth & develop of expanded services to include participation in design, development, and staffing of new Community-Based clinics and Health Care Centers. Provides expertise in population-based health care guided by data from VSSC, electronic Quality Measure Report, Combine Measure Report, Outpatient Sites of Care Report, and the External Peer Review Process (EPRP). Offers guidance related to the VSSC Almanac reports including the +FOBT to follow-up Colonoscopy, Diabetic Reports by Cohort, Hypertension Cohort Reports, Primary Care Panel Overview, CAN scores, and Immunization Reports for panel health care management. Plans, leads, and participates in Primary Care Leadership Retreats. Influences organizational mission and healthcare with responsibility for performance outcomes related to HEDIS measures, Customer Service & Patient Experience measures, Ambulatory Care Sensitive Conditions, Patient Aligned Care Team measures, Joint Commission regulatory standards, RN turnover, and All Employee Survey scores. Actively participates in accreditation and regulatory reviews including The Joint Commission and OIG CHIP reviews. Responsible for ensuring continuous readiness, developing action plans, tracking compliance, and reporting updates on regulatory findings related to Primary Care nursing service. Significant travel responsibilities to ensure regular visits at each Community-Based Outpatient Clinic so staff feel a leadership presence and have their voice heard. Ensures Intentional Rounding is a priority and leadership expectation. The CBOC ANE has continuing responsibility for the following duties: Planning work to be accomplished by subordinates, setting priorities, and preparing schedules for completion of work. Assigning work to subordinates based on priorities, selective consideration of difficulty and the requirements of the assignments, and the capabilities of subordinates. Evaluating the performance of subordinates and addressing employee conduct Making recommendations for appointments, advancements, or reassignments of subordinates Collaborating with Human Resources for recruitment & retention Giving advice, counsel, coaching, or instruction to subordinate personnel on work and administrative matters. Hearing and resolving complaints of subordinates and referring more serious complaints not resolved to higher level supervisors. Recommending and/or taking disciplinary action where appropriate; and Vetted: 01/24/2024 Identifying developmental and training needs of subordinates and providing or making provisions for such development and training Presents to Resource Management Board for additional positions to support the mission of primary care, women's health, and employee health. Intentional Rounding including regular travel to all sites of care. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education Accreditation Requirement. Nursing education must be from a school of professional nursing accredited by one of the following accrediting organizations at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN), which accredits all levels of nursing programs, including clinical doctorate, masters/post master's certificate, baccalaureate, associate and diploma nursing education programs. (Note: The National League for Nursing Accrediting Commission/NLNAC changed its name to ACEN in May 2013; OR The Commission on Collegiate Nursing Education (CCNE). The CCNE is an accrediting arm of the American Association of Colleges of Nursing (AACN) and accredits nursing education programs at the bachelor and master's level. In cases of graduates of foreign schools of professional nursing, who possess a current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing to warrant appointment as a Nurse who has completed an associate degree level Nursing education program. In order to grant credit towards an appointment based on foreign nursing education higher than the associate degree level, formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD) must be obtained. Degree in a Related Field. A graduate level or doctorate degree in health care/management related fields from an institution accredited by the State and approved by the Department of Education at the time the candidate completed the program may be credited as specifically indicated in grade requirements. The graduate level or doctorate degree and coursework must provide evidence in support of the nursing role and duties associated with the assignment and grade level. Foreign education programs/degrees are not creditable as related degrees. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Master's degree in nursing and four years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. OR Master's degree in a related field and a bachelor level degree in nursing and four years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. OR Doctorate in Nursing and three years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. OR Doctorate degree in a related field and a master or bachelor level degree in nursing and three years of professional nursing experience, one of which is specialized nursing experience equivalent to Nurse III and meets all dimension requirements for Nurse IV. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The work is principally sedentary, however, duties routinely requires walking, standing, bending, carrying, etc. in a normal office/medical center environment
