The VA Sierra Nevada Health Care System (VASNHCS) Community Living Center (CLC) Registered Nurse (RN) is a professional nurse who provides direct care to Veterans with a variety of chronic medical conditions. The CLC is a 60 bed unit, and RNs care for Veterans with an array of conditions including short and long stay rehabilitation needs, SCI, dementia care respite care and hospice. The CLC RN provides 24/7 care in a homelike environment.

Duties

The CLC RN assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Duties and responsibilities include but are not limited to the following: Delivers fundamental knowledge-based care. Demonstrates real-world application of the nursing process. Uses collaborative strategies with all VASNHCS departments in order to maximize patient care. Provides safe and cost-effective care. Communicates with physicians and other clinicians. Delegates staff within their units on a continual basis (24/7). Validates the daily patient classifications, and works with other Nurse Managers to identify and share resources. Works to improve nursing practice and patient care. Identifies real or potential safety concerns. Demonstrates professional behavior in interdisciplinary relationships. Demonstrates leadership in delivering and improving holistic care. Demonstrates the ability to identify an issue, analyze it, and develop and implement interventions. Uses collaborative strategies with all VASNHCS departments in order to maximize patient care. Provides effective, compassionate delivery of health services congruent with the health care system mission and vision. Delegates and sets priorities for patient care. Assess and provides feedback on the performance of others. Develops and manages collaborative partnerships with physicians and other clinicians. Participates on service and medical center committees and task forces to improve nursing practice and patient care. Utilizes creative problem-solving methodologies to resolve problems. Shares knowledge with others to enhance nursing practice. Responsible for customer service/patient satisfaction, contributing to a safe work environment, and contributing to healthy interpersonal relationships. Addresses ethical issues that impact patient and staff. Analyzes ethical issues, applying appropriate theoretical principles and provides guidance. Identifies and corrects real or potential safety concerns. Demonstrates professional behavior in interdisciplinary relationships. Effective verbal and written communication skills. Maximizes the use of computerized data validation and communication to enhance patient care delivery. Uses critical thinking skills and sound judgment to ensure positive outcomes demonstrate ethical practice and participate in performance improvement activities to improve quality care. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. Demonstrates leadership that is sustainable regardless of the role. Serves as a mentor and consultant to healthcare staff involved in patient care. Provides leadership to promote an effective, compassionate delivery of health services congruent with the health care system mission and vision at the facility level. Assigns and/or delegates work to other staff members based on priorities, selective consideration of the difficulty and the requirements of the assignments, and the capabilities of the staff members. Gives advice as a subject matter expert on work and practice matters. Identifies developmental and training needs for the facility, service, or program and providing or making provision for such development and training. Responsible for customer service/patient satisfaction, patient and employee safety, employee relations, standards of care and practice and the work environment across multiple services. Develops and manages collaborative partnerships with physicians and other clinicians. Qualifications

Basic Requirements: Citizenship. Non-citizens may only be temporarily appointed when it is not possible to recruit qualified candidates in accordance with 38 U.S.C. 7407(a). English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education. Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level bridge program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the bridge program. Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program. (Reference VA Handbook 5005, Appendix G6). OR Foreign Nursing Education In cases of graduates of foreign schools of professional nursing, possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD). Licensure. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry step of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.) Grandfathering Provision: All persons employed in VHA in this occupational series and performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/ certification. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply for Nurses: Who are retained under this provision and subsequently leaves the occupation, loses protected status and must meet the full VA qualification standard requirements in effect at the time of re-entry to the occupation Initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification and meet all the basic requirements of this qualification standard, must maintain the required credentials as a condition of employment in the occupation Who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard Grade Determinations: The following Scope, Education and Dimension criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade. Grade/Level Scope Education Nurse I, Level I Delivers fundamental, knowledge-based care to assigned clients while developing technical competencies. An Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required. Nurse I, Level II Demonstrates integration of biopsychosocial concepts, cognitive skills and technically competent practice in providing care to clients with basic or complex. An ADN or Diploma in Nursing AND 1 year of specialized nursing experience equivalent to Nurse I, Level 1 ;OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required. Nurse I, Level III Demonstrates proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I, Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I, Level 2; OR a Master's degree in nursing (MSN) and no additional professional nursing experience; OR a Master's degree in a *related field with a BSN and no additional professional nursing experience. Nurse II Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. A BSN with 2 years of professional nursing equivalent to Nurse I, Level 3; OR an MSN with one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Master's degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Doctoral degree in Nursing with no professional nursing experience; OR a Doctoral degree in a *related field with a BSN with no additional professional nursing experience. Nurse III Executes position responsibilities that demonstrate leadership, experience and creative approaches to management of complex client care beyond the immediate practice setting. MSN and 2 years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Master's degree in *related field with BSN and two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in Nursing with and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III. *Note: Foreign education programs/degrees are not creditable as related degrees. Preferred Experience: Experience as a nurse in a long term care facility and at least one (1) year experience as a Registered Nurse. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. Physical Requirements: As outlined in U.S. Office of Personnel Management, Certificate of Medical Examination, Form OMB No. 3206-0250 including intermittent manual lifting or carrying up to 35 pounds, sitting, standing, walking, kneeling, bending, stooping, twisting, pulling, pushing, simple grasping, fine manipulation (including keyboarding), and reaching above shoulder for up to 12 hours per day. Mechanical lifting devices will be used for weights exceeding 35 pounds.

Education

    • -The graduate level or doctorate degree and coursework must provide evidence in support of the nursing role and duties associated with the assignment and grade level.
    • Note: Foreign education programs/degrees are not creditable as related degrees.
