This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Duties include but are not Limited to: Responsible for direct linkages between the Veteran and the local housing authority and is responsible for direct referrals for Housing Choice Vouchers as it pertains to the HUD-VASH program. Works closely with the VA Medical Center's homeless programs, residential and community care programs and transitional living facilities to ensure that coordination between these entities is facilitated and Veteran's direct care, referral and follow-up is contiguous and based on a continuum of services. Establishes implements and maintains referral and screening procedures for potential HUD-VASH Veteran participants that meet national policy as well as the needs of medical center mental health services and residential programs. Referral, screening, and admission criteria recognizes program eligibility, prioritization of subsets of the homeless Veteran population, requirements of the housing authorities, and other policy, regulation, or law pertaining to program administration. Conducts and participates in outreach activities including field interviews, assessment and referrals for homeless Veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers, or outpatient clinics. Evaluates the Veteran's situation, abilities and capabilities and arrives at a reasoned conclusion including an assessment of vulnerability and prioritization for admission. Establishes procedures and coordinates the screening and admission process for the HUD-VASH program by developing and maintaining criteria that recognizes program eligibility, prioritization for program acceptance, and requirements of the housing authorities. Develops comprehensive treatment plans with the Veteran and implements the steps to assist the Veteran in meeting set goals. Makes routine visits, based upon acuity, to each Veteran in residence to assess their environment and provide on-going clinical services. Provides consultation and assistance to HCHV program including NEPEC data reporting as required. Is trained on all aspects of the HCHV program and provides coverage if necessary. Provides social work consultation to other professional staff both inpatient and outpatient. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. (1) Experience/Education. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. (2) Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Applicants applying at the GS-12 grade level must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Examples of specialized experience include: Assess and document identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence. Maintain patient privacy and confidentiality per policies, handbooks or directives; Independently assesses the psychosocial functioning and needs of patients and their family members, identifying the patients strengths, weaknesses, coping skills, and psychosocial acuity. Provides case management and care coordination to facilitate appropriate delivery of health care services. Conducts timely assessment of at-risk patients in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. Provide interventions independently with patients and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Preferred Experience: Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
