The clinical pharmacy specialist in Tele-EC is the provision of evidence-based pharmaceutical care services through direct and indirect interaction with patients and providers. VISN 17 Tele-EC Clinical Pharmacy Specialists (TECCPS) are highly trained medication management experts and consultants in highly complex, clinical scenarios, collaborating with healthcare providers to ensure safe, appropriate, and cost-effective use of medications in partnership with the VISN 17 Tele-EC Program (TEC).

Duties

Clinical Collaborates with Tele-EC physician team to develop, implement, and maintain systems that promote desirable clinical outcomes through use of safe, cost-effective drug therapies. Reviews and evaluates medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes. The TECCPS will document and communicate findings in the electronic health record. Utilizes motivational interview skills with patient, family, caregiver, and other healthcare professionals to collect patient-specific data, conduct medication history taking to identify problems and individualize care to complement patient's medication therapy history, formulate a pharmaceutical care plan, and identify additional data needed for medication review assessment and medication reconciliation as part of component provider duties. Designs pharmaceutical care plans if needed and documents interventions performed in electronic health record, allowing for seamless transitions of care and effective communication within the patient's healthcare team. Makes dose adjustments and recommendations based on objective laboratory findings, expertise in the principles of clinical pharmacokinetics and pharmacodynamics. Orders, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Follows-up with patients on lab or test results to discuss the plan of therapy (i.e., changes in medication therapy, monitoring and evaluation, additional testing requirements). Manages acute and chronic disease states under Scope of Practice. The TECCPS will assess the Veteran, order appropriate labs, analyze, and prescribe appropriate medication therapy for optimal outcomes and improve patient access to care. Education Participates in the training and orientation of pharmacy students, pharmacy residents, pharmacy staff and allied healthcare professionals as necessary. Provides training and education related to medications used during emergency response to first responders (when requested). Serves as an authoritative information source on medication and their uses. Provides both written and oral drug information to physicians, nurses and other healthcare professionals as well as to patients and their caregivers. Engages in drug information services to healthcare providers informally and via formal requests. The TECCPS utilizes the most recent literature to draw a clear, concise conclusion applicable to effective disease management and pharmacotherapy. Maintains and improves professional competency by keeping up with information on new medications, new medication therapies, and innovations in pharmacy practices. The TECCPS continually applies current drug information and program techniques in their daily practices. Administrative Responsible for the daily operation status of various aspects of the pharmacy programs. The TECCPS prioritizes work and makes administrative decisions instances where immediate decisions are required. Pharmacy Service Policy and Procedures, VA Regulation and state and federal Pharmacy laws will serve as operational guidance. Exhibits leadership skills by active involvement in committees and workgroups as needed/as interested. Participates in quality improvement committees and regulatory compliance activities designed to evaluate and improve patient safety, quality measures/metrics and outcomes as pertains to the VISN 17 Tele-EC Program, included but not limited to creation of order sets or other techniques to aid in expedited, safe and proper selection of evidence-based and/or guideline-directed therapies. Qualifications

Basic Requirements: Citizenship. Citizen of the United States. (Noncitizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.) Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (Must submit a copy of Transcripts) (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Grade Determinations: Clinical Pharmacy Specialist GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Demonstrated KSAs. Pharmacists assigned to this position must also demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Greater than 18 months experience in remote/Telework Pharmacy and use of digital interface to interact with patients Greater than 18 months experience in private sector/outside Veterans Affairs system Experience in development and implementation of programs involving medication therapy management Experience in initiatives targeted toward promoting patient behavioral change Experience in education and teaching, preferably in a pharmacy program Experience in provision of direct patient care References: VA Handbook 5005/55 Part II Appendix G15 dated June 7, 2012 The full performance level of this vacancy is GS-13. Physical Requirements: The physical demand of the work includes near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20; specific visual requirement, ability to read fine print on labels; depth perception; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); clear speech; and emotional stability.
