The Nurse Practitioner has leadership in delivering and improving holistic care through collaborative strategies with others. They have the ability to manage complex patient situations. They are responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care at Eureka Community-Based Outpatient Clinic (CBOC).

Duties

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Major duties include, but are not limited to: Identifies physiological, psychological, social and environmental variable affecting the functional status of the older adult and palliative care patient. Identifies current coping mechanisms an support systems and their effectiveness. Identifies current level of knowledge and skill of patient/significant other(s) in management of health care. Identifies needs for possible discharge planning and appropriate referrals. Documents on approved VA forms/computer programs Determines nursing and medical diagnoses based on analysis of assessment data. Plans interventions which promote health. Develops teaching plans based on assessment of individual needs. Plans alternative interventions for medicine/surgical patients with unique needs. Initiates patient care conferences with staff. Assures implementation of the plan of care. Maintains a high standard of practice in giving direct care to patients with complex needs. Participates in interdisciplinary patient care rounds. Assists staff as needed in providing care for identified patients and families. Provides individualized instruction or directs nursing staff in teaching specific content/skill to geriatric patients/significant others. Anticipates possible discharge needs and assists patient/significant others and multidisciplinary teach in achieving goals for discharge. Prepares professional clinical documentation of patient care. Evaluates services rendered and the patient's response to the plan of care. Mutually, with professional colleagues, monitors the patient response and makes needed revisions. Provides professional leadership regarding nursing standards and services. Participates in established continuing improvement per medicine/surgical policies. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education. A post-master's certificate or master's or doctoral degree as a Nurse Practitioner (NP). (NOTE: the post-master's certificate is formal education awarded from an acceptable school of professional nursing and is not considered board certification). Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time I completed the program: The Accreditation Commission for Education in Nursing (ACEN) OR the Commission on Collegiate Nursing Education (CCNE); OR In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. License. All NPs must possess and maintain an active, current, full and unrestricted license to practice as a NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Prescriptive privileges (Nurse Practitioner Furnishing Number) with a current DEA Licensure from the United States Drug Enforcement Administration for scheduled II-V controlled substances with demonstration of meeting State Board of Nursing requirements to prescribe controlled substances. Basic Certification: All NPs must maintain full and current board certification in an area of clinical specialty from one of the following: The American Nurses Credentialing Center (ANCC); OR The American Academy of Nurse Practitioners Certification Board (AANPCB); OR Other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared. Basic certification as a nurse practitioner is a condition of employment and qualifying factor for the specific assignment. NPs are credentialed and privileged to provide care based upon scope of practice, formal graduate nursing educational preparation, certification and patient population. It is the focus of care based on patient care needs, not the care setting, that defines the provider's scope of practice and basic certification required. NOTE: Grandfathering Provision. All persons employed in the Veterans Health Administration (VHA) in this occupational series and NP assignment that are also performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. Grade Determination. In addition to the basic qualification requirements, the grade level of an NP is based on a combination of education and experience. Beyond the minimum education requirements, grade determination for appointment, advancement and promotion will be based on the following specific requirement for an NP. The level of practice at which an NP may perform is based upon a combination of many factors. Beyond the minimum educational requirements, grade determination, upon appointment and advancement, is dependent upon aspects such as continued professional development (formal and/or informal education), clinical performance, judgment, outcomes (at the service level and beyond), leadership and interpersonal skills that influence, inspire and motivate others within their program/service or beyond. As the NP acquires the knowledge, skills and judgment that accrues with experience, the privilege of increased authority for patient care and service in administrative roles is expanded as applicable. Grade/Level Scope Education Nurse Practitioner II Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. A post-master's certificate OR master's OR doctoral degree as a Nurse Practitioner. Nurse II is considered the entry grade level for NPs; no experience is required. Nurse Practitioner III Executes position responsibilities that demonstrate leadership, experience and creative approaches to management of complex client care. A post-master's certificate OR master's OR doctoral degree as a Nurse Practitioner AND One year of specialized NP experience equivalent to the Nurse II grade level. Salary for this position ranges dependent upon applicant qualifications Grade 2: $152,371-$202,652 annually Grade 3: $166,084-$220,886 annually Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6a Advanced Practice Nurse (Nurse Practitioner) Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
