This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Michael E Debakey VA Hospital is looking for a full-time Physician Assistant to provide care to veterans in our Primary Care Clinic/Women's Health. Incumbent will perform outpatient duties and management of scheduled and unscheduled patients, Monday through Friday, under the Patient Aligned Care Team (PACT) model accountable for adhering to established policies and standards of care/standards of practice in evaluating, tracking, and following up on patient care.
Major Duties (To include but not limited to):
Performing medical histories and physical examinations for women Veterans.
Managing medical problems to include writing orders for medications for stable medical conditions and treating minor illness; ordering laboratory tests; ordering imaging tests; adhering diets and over-the-counter medications and supplies; reviewing diagnostic tests; reports and consult.
Observing and recording patient's medical progress, updating and summarizing medical records, and changing orders when appropriate.
Write prescriptions in accordance with Federal and State law and as allowed by the Scope of Practice, that support breast cancer risk prevention.
Acting on breast cancer preventative measures according to standards of evidence-based care.
Performing medication reconciliation
Independently plans and delivers technically competent care to patients with complex needs.
Safely and effectively uses equipment required for patient care.
Reports equipment in need of repair.
Recognizes subtle abnormal signs and symptoms and/or changes in patients' condition and responds appropriately.
8. Documents process.
Documentation is technically informative, consistent, accurate, concise, and timely manner.
Evaluates and documents patient response to interventions and progress towards attainment of expected outcomes.
Completes notes and/or uses addenda to avoid unsigned progress notes.
Revises plan of care according to evaluations of patient responses.
Uses the Computerized Patient Record System (CPRS).
9. Manages care of patients.
Provides direction to staff in meeting patient care needs.
Ensures that assigned delegated tasks are completed by team members.
Organizes and completes own assignments in a safe and timely manner, assisting team members as necessary.
Organizes and prioritizes in order to meet changing patient care needs and staff assignments.
Assigns patient care responsibilities based upon patient needs and competencies of staff.
10. Demonstrates grade-appropriate and consistent critical thinking skills.
The Physician Assistant can recognize the patient has a problem.
The Physician Assistant manage the problem safely and effectively, recognizing his/her scope of practice.
The Physician Assistant has a relative sense of urgency.
The Physician Assistant takes the right action for the right reason (Del Bueno).

Physician Assistant Basic Requirements
Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~
Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~
Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
Preferred Experience:
- Performing medical histories and physical examinations.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Physician Assistant-I (PA-I)
Experience and Education: None beyond the basic requirements. This is the entry level for PAs.
Physician Assistant-II (PA-II)
Experience and Education: The PA at this grade is at the intermediate grade level and has had progressively increasing responsibility and experiences.
Two years of successful practice as a PA.
OR Successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency.
OR A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education.
Physician Assistant-III (PA-III)
Experience and Education: PAs at the PA-III grade are at the full performance level and lead and organize patient care independently, practice within multi-disciplinary teams, and coordinate all aspects of patient care across the health care system.
Three years of successful practice as a PA.
OR Successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency and one year of professional practice as a PA.
OR A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education and one year of professional practice as a PA.

The full performance level of this vacancy is 3. The actual grade at which an applicant may be selected for this vacancy is in the range of 1 to 3.
Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients
who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
