This position provides comprehensive nursing services to individuals, families, and communities within a designated geographical area. Services are provided to all age groups with focus on health promotion and disease prevention The health program is carried out in homes, offices, clinics, schools and the community.
Duties
Conducts community assessments to identify physical, mental, social and/or environmental deviations from the normal, responses and attainments of outcomes, using nursing skills of assessment, nursing diagnosis, planning intervention and evaluation.
Assesses patient needs and then develops plans of care. Consults with health care team members, including participating in case management with health care providers.
Counsels patients and/or families in the home, community, school and clinic regarding acute and chronic conditions and the application of health promotion and disease prevention practices.
Participates in Performance Improvement activities, facilitates and promotes Government Performance Results Act activities, and is involved in facility accreditation activities.
Qualifications

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.
BASIC REQUIREMENT(S):
Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, the position requires experience in Community Health Nursing.
In addition to the above requirements, applicants must meet the following requirements based on the grade level you are applying to:
GS-09: Possession of a master's degree in professional nursing; or
At least one full year of professional nursing experience in Community Health Nursing (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.
Examples of GS-09 work experience include: At least one full year of professional public health nursing experience (equivalent to the GS-7 grade level or above) working with individuals, families, and communities implementing and supporting community care coordination; and interacting with health care system providers on referrals and transitions in care; and collection and analyze data as part of community assessment, implementing case management processes and mentoring colleagues.
GS-11: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; or
At least one full year of professional nursing experience in Community Health Nursing (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.
Examples of GS-11 work experience include: At least one full year of professional health nursing experience (equivalent to the GS-9 grade level) applying nursing theory and public health science to preserving and promoting the health of the population with the nature of the practice being continuous and comprehensive; development and implementation of needed health services in the community to address the well-being of the population and serving as an advocate and facilitating change; Provision of public health program activities which are based on the community needs, program resources, and establishing goals and objectives; and, identification of vulnerable groups in a community by assessing the needs of the population and tailoring services to the special needs of high risk groups.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
