The Franklin Delano Roosevelt Hospital VAMC is currently recruiting for a Veteran Justice Outreach Program Coordinator. The responsibilities are to build new relationships and expand existing ones, including community treatment programs, court judges, court personnel, district attorneys, public defenders, parole and probation, private attorneys, the federal legal system and VISN regional Council to assist and serve Veterans involved in the legal system.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Duties: The Veterans Justice Outreach Coordinator primary responsibilities are to provide psychosocial assessments, case management services, continuous quality improvement activities, evaluation/consultation, placement services, and referral and linkage to various community agencies. For Veterans who are within six months of release from correctional institutions (city, county, state and federal) and/or are being monitored by the courts, including treatment or problem-solving courts. The VJO Coordinator provides supportive case management for re-entry veterans for up to four months post-release. The Veterans Justice Outreach Coordinator collaborates with other Veterans Justice Outreach clinicians to support engagement in care for eligible Veterans recently discharged from correctional institutions. Contacts include providers who serve Veterans by contract or formal agreement with the VA, government staff, social, financial, legal, health and mental health agencies, Veterans and/or their families/relatives, Veterans' conservators, friends, employers, landlords, police, attorneys, judges, and probation and parole officers. The Veterans Justice Outreach Coordinator collaborates with state and community (including HUD Continuums of Care homelessness prevention efforts) and VA CHALENG re-entry initiatives. The incumbent is required to conduct active outreach to Veterans involved with the justice system, including those who may be participants, or candidates for participation, in treatment court programs, including Veterans Courts. This outreach may also include field interviews, assessments and referrals for Veterans contacted in community settings, courts and/or prior to release in jails or prisons, as well as those referred by VA residential programs, medical centers, or outpatient clinics. Advanced clinical training, insight and experience is utilized to interpret data and identify viable treatment options. The Veterans Justice Outreach Coordinator assesses high risk factors, acuity, and the need for services. This Veteran Justice Outreach Coordinator can serve Veterans who tend to have frequent and severe crises, lack family or an adequate community support network, be poor at self-monitoring, frequently fail to comply with instructions and treatment, or have significant deficits in coping skills and require continuing professional psychosocial support. The Veterans Justice Outreach Coordinator provides referrals and linkage to VA medical and mental health treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. If appropriate, the Social Worker provides referrals or consults to other VA programs, including Grant and Per Diem programs, Health Care for Homeless Veterans contract residential treatment, Domiciliary or Psychiatric Residential Rehabilitation Programs, or Compensated Work Therapy or Supported Employment programs. The Veterans Justice Outreach Coordinator participates as an active member of the court treatment team, providing or coordinating the provision of clinical treatment using evidence-based models appropriate to the justice-involved Veteran population, including, for example, Motivational Interviewing, Seeking Safety, or other evidence-based interventions. The VJO Program Coordinator Social Worker assists the Veteran in services stabilization by consulting and intervening when problems arise with service providers and assists in priority management/pace of services/integration of Veteran in program with Veteran and program staff by participating in treatment team meetings. The Veterans Justice Outreach Coordinator captures all clinical service activities as required by Homeless Care Line programs, including medical charting and completion of appropriate VA NEPEC forms. VJO Program Coordinator Specialist performs other related duties as assigned. The Veterans Justice Outreach Coordinator interacts with both the legal and the VA systems, which both have very complex structures with a high number of policies and standard operating procedure. The VJO Program Coordinator coaches' members of the legal system on how to negotiate the VA systems and he/she coaches members of the VA system on how to negotiate the legal system. In addition, the he/she coaches the Veteran on how to negotiate between the two systems. It is essential to high quality care that both systems work together in a seamless way as possible. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. Loss of Licensure or Certification. Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). NOTE: If social workers covered under the grandfathering provision of the 1991 Federal law regarding licensure or certification of VHA social workers leave the GS-0185 social work series, they lose the grandfathering protection. If they choose to return at a later date to the GS-0185 series, they must be licensed or certified to qualify for employment as a social worker. Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.
