The incumbent serves as a Management & Program Analyst, providing support to Primary Care Service operations at Minneapolis Care System (HCS) within the VA Midwest Health Care Network, VISN 23. This position reports to the Administrative Officer/Health System Specialist (Primary Care) and has primary responsibility as the Patient Centered Management Module (PCMM) Facility Coordinator for all Primary Care services with the Minneapolis and associated Community Based Outpatient Clinics (CBOCs).

Duties

Major Duties include but are not limited to: Management and oversite of the PCMM Web Program, which involves a complex, nationally implemented, stand-alone computer program that is designed to monitor and manage primary care services throughout the VA system. Analyze and interpret applicable PCMM and HBPCMM data to compile regular primary care panel reports which include information regarding the capacity of all primary care providers' panels. Complete comparative analysis of VSSC reports or data sets with local PCMM data to identify data variance and ensure data integrity. Ensure facility PCMM standard operation procedures (SOP) or Directives are in place and current and meet all local requirements to meet goals and objectives. Establish and manage PCMM Web position set up; staff and FTE assignment, as well as patient assignments for all primary care locations. Meet regularly with facility primary care leadership and group practice manager to review provider panel management data and participate in monthly national calls. Communicate regularly with clinic site leadership at all CBOCs and sites of care to ensure timely responses and resolution to any PACT and/or PCMM issues. Utilize data to analyze and evaluate the performance, effectiveness, and efficiency of the PCMM Web program, often developing databases and reports if needed. Analyzes data generated from a variety of sources (e.g., Pyramid Analytics, data warehouse, VISTA, CPRS, Sail) to identity process improvement opportunities.. Oversees data and information management needs to include monitoring, analysis, interpretation, and presentation of data. Reviews new agency directives, guidelines, and other publications to identify those that have an impact on local clinical policies and processes. Utilizes office skills, prepares presentations of information using such applications as Microsoft Word, Excel, PowerPoint. Facilitate courses or presentations related to all aspects of PCMM Web and gathers information to determine training needs, analyzes information obtained, and submits recommendations to supervisor. Develop and/or modifies procedures, courses, training guides and plans as appropriate to effectively meet the training needs, often assessing their quality and effectiveness. Performs management reviews to determine compliance with agency and location regulations, policies, procedures, sound management practices, and effective utilization of staff. Prepare new local policy and reviews established local policy in accordance with VHA Directive and delegations of authority and advices of potential impact on the organization. Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/07/2024. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. For a GS-07 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. You may qualify based on your experience and/or education s described below: GS-07: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-05 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Analyze and collect data from various sources such as clinical programs and systems: Preparing projects data results in various formats including creating charts, graphs, reports, spreadsheets, etc; Proven use of Microsoft software for word processing, Email, presentations, spreadsheets, and databases; Familiarity with interpreting and ensuring compliance with laws, regulations, and policies. (This experience must be well documented in your resume or application package). Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed one full year of graduate level education or have superior academic achievement ***. OR, Combination: Applicants may also combine education and experience to qualify at this level. You must have an combination of specialized experience and education beyond bachelor level. **S.A.A. is based on (1) class standing, (2) grade-point average, or (3) honor society membership. Class standing -- Applicants must be in the upper third of the graduating class in the college, university, or major subdivision, such as the College of Liberal Arts or the School of Business Administration, based on completed courses. Grade-point average (G.P.A.)-- Applicants must have a grade-point average of: 3.0 or higher out of a possible 4.0 ("B" or better) as recorded on their official transcript, or as computed based on 4 years of education, or as computed based on courses completed during the final 2 years of the curriculum; or 3.5 or higher out of a possible 4.0 ("B+" or better) based on the average of the required courses completed in the major field or the required courses in the major field completed during the final 2 years of the curriculum. Grade-point averages are to be rounded to one decimal place. For example, 2.95 will round to 3.0 and 2.94 will round to 2.9. Election to membership in a national scholastic honor society -- Applicants can be considered eligible based on membership in one of the national scholastic honor societies listed by the Association of College Honor Societies. GS-09: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization.
