Duties

Licensed Vocational Nurses assist the Registered Nurse in the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management and supporting factors that impact customer satisfaction. Considers all characteristics of the individual including age and life stages, state of health, race, culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. The incumbent must provide patient care in a timely manner, recognizing and responding to physical, psychological, spiritual, and emotional needs of the patients. Demonstrates knowledge and skills to perform more complex practical nursing assignments such as complicated sterile dressing changes/monitors regulates IV fluids and give specialized care to comatose patients. Willing to work cooperatively as a member of a team in all assignments. Incumbent duties will be determined by assignments and competencies. Makes caring for the veteran his/her priority while in the work setting and demonstrates customer service principles in all aspects of work. The Licensed Vocational Nurse is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life. The Licensed Vocational Nurse requires a general review of their work by an Registered Nurse for appropriateness and conformity with license, policies and procedures. The Licensed Vocational Nurse assigned in the in-patient care areas demonstrates active collaboration and communication with the Registered Nurse, Patient, Primary Care Provider, and other care team members. It is also imperative that the Licensed Vocational Nurse establish a positive rapport with family/caregiver, internal and community-based services and disciplines involved in providing care to the patient. The Licensed Vocational Nurse assigned in the out-patient care areas demonstrates active collaboration and communication with Teamlet (Patient, Primary Care, Provider, and Registered Nurse Case Manager) and Larger Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient). The position requires flexibility in schedule and assignments. Work Schedule: Fixed Shifts; Sunday, Monday, Tuesday 12 hour shift from 7:00pm to 7:30am with a 8 hour night shift ever other Wednesday. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in both spoken & written English as required by 38 U.S.C. 7402(d), & 7407(d). Licensure: Full, active, current & unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. Physical Standards. See VA Directive and Handbook 5019. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant. Exceptions may include: 1) Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure; 2) VHA employees converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, or under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; & for their future personnel actions under this authority. 3) Individuals granted a license by a jurisdiction that does not require graduation from an approved school. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation & meet the criteria). Preferred Experience: Two to three years experience in caring for patient with mental health needs and or Residential nursing care experience. Ability to collaborate and communicate effectively with multidisciplinary teams both within and outside Mental Health. Grade Determinations: GS-3: None beyond the basic requirements. GS-4: (a) Six months of qualifying experience as an LPN/LVN; or (b) Graduation from an approved school & one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure & sterile techniques & procedures; or (c) Graduation from an approved school of at least 24 months duration. GS-5: (a) Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent. (b) Demonstrated knowledge & ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Works with the RN and/or MD/DO to appropriately orient/train less experienced LPNs/LVNs and/or Nursing Assistants (NAs)/Health Technicians (HTs) in relation to acceptable standards of practice in promoting optimal patient care delivery. Provides effective education to patients and/or family members in relation to common disease processes, medication, and /or prescribed treatment regimes. (c) Demonstrated ability to serve as a responsible member of the nursing team & interact in an appropriate manner with patients, family members, professional & other supportive personnel involved in the delivery of patient care, incorporating acceptable, established customer service standards into practice. (d) Knowledge & skill sufficient to prepare, administer, & appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies & procedures. (e) Knowledge & ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, & assist in resuscitation procedures in cardiac and/or pulmonary arrest. (f) Recognizes & appropriately responds to breakage/malfunction or loss of equipment, safety hazards, & supply deficiencies, promptly reporting to appropriate personnel for corrective action. GS-6:(a) Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent. (b) Technically proficient in initiating, performing & completing assigned duties in providing care to variable patient populations. (c) Knowledge & ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order & sequence of procedures & treatments; & to accurately recognize, report & record relevant patient information. (d) Ability to observe, identify & respond to the patient's needs for care, including medication, equipment-assisted care & patient/family education. In organizing & delivering care, the LPN/LVN recognizes & considers emotional, cultural, spiritual, socio-economic, & age-related factors. (e) Prepares & administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) & performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, & reporting any deviations from normal to RN or MD/DO. (f) Knowledge & ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, & initiate appropriate emergency interventions as directed. (g) Knowledge & understanding of human behavior, patient motivations & reactions to situations, & ability to appropriately utilize this knowledge in working effectively with patients, family members, & other staff. (h) Establishes constructive relationships with individual patients & their families to elicit feelings & attitudes, & to promote positive relationships, communication & socialization skills. Fosters an environment of respect for individual patient & family rights to privacy & dignity in all aspects of care delivery. Effectively incorporates knowledge & understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers. (i) Knowledge & skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing & handling specialized instruments or other specialized equipment, & monitoring the patient's condition before, during, & following the procedure. Serves as a preceptor in orienting, educating, & training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures. (j) Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve & advance nursing practice. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). The actual grade at which an applicant may be selected for this vacancy is in the range of GS-3 to GS-6. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. Incumbent may carry and lift light items weighing less than 15 pounds.
