The Caregiver Support Program (CSP) is one of the Veterans Health Administration's (VHA) most rapidly growing service areas, as a result of infrastructure modernization, programming innovation, and expansion of population-served by the Program of Comprehensive Assistance for Family Caregivers (PCAFC) and expansion of Program of General Caregiver Support Services (PGCSS) under MISSION Act. CSP expansion puts the VHA out front of the healthcare industry as a healthcare system pioneer.
Duties
DUTIES:
Develops, coordinates, and implements a comprehensive outreach initiative designed to facilitate the delivery of services to caregivers and their Veterans. This requires the incumbent to identify the needs and requirements of the Caregiver population, services they are not able to access on their own, partners in the community who are willing to provide services or will jointly develop services. Serves as the first point of contact and subject matter expert regarding questions about the program, coordination of referrals, building community partnerships, caregiver utilization, data reporting, and any other administrative support tasks required to support the program at the facility level.
Provides resources and referrals to Veterans and caregivers, who are seeking caregiver support services, regarding program services, concerns, appeals of decisions and ratings. Obtains as much information as possible, provides information, resolves problems, and makes decision regarding caller referral based on specific called needs and location. As a referral and follow-up specialist, reviews Veteran and caregiver referral and resource utilization, participates in caregiver team meetings, and ensures continuity of care by maintaining communication and documentation.
Monitors and evaluates caregiver activities (utilization of referrals/resources, trainings, appointments with the Veteran, caregiver strain, etc.), assessments, enrollment processes, and makes recommendations for improvements for the Caregiver Support Program.
Identifies and builds relationships with community partners and stakeholders, such as Veteran Service Organizations, faith-based organizations, United Way, fraternal organizations, and other nonprofits, to enhance and improve referrals and resources for caregivers and their Veterans. Maintains a comprehensive list of community resources and services for the program. Collaborates with CSP PSA's across the VISN and VA system.
Monitors the MISSION Act and provides updates to local VA leadership, VA providers, community providers and Veteran Service Organizations.
Accepts telephone calls from Caregiver Support staff in the field, other VA staff, and/or community agency staff, regarding program inquiries, program concerns, etc. Incumbent obtains as much information as possible, provides information and resolves problems independently when possible, prioritizes, and makes decisions regarding caller referral based on specific caller needs. The incumbent ensures timely response to team members that potentially have an emergent or urgent care need through prompt identification of the problem and appropriate team referral. Interacts with visitors and co-workers in a courteous, tactful, helpful manner, exercising mature judgment and diplomacy.
Prepares materials for trainings, presentations, and "National Family Caregiver Month" information tables, and Caregiver Support Advisory Council meetings. Also schedules conferences and meetings, including arranging logistics, preparing agendas, and notifying participants.
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 03/06/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-07 position you must have served 52 weeks at the GS-06. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.
You may qualify based on your experience and/or education as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade (GS-06) in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Using a computer to analyze or process incoming forms, records, or other documents; determine when new procedures are needed, periodic reviews of clinical charts to ensure completion of necessary documentation , fact find and provide analysis of issues in accordance with accepted office policies and organizational concepts, preparing yearly funding requests, maintain record of all associated reimbursement costs, Enters pertinent data into an automated tracking system and maintain program specific data.

Physical Requirements: The work is primarily sedentary and is performed by sitting, standing, bending, and stooping. The employee is required to obtain records from files, and carry files, manuals, and other light objects. Occasional travel is required to provide support at conferences, workgroups, programs or meetings
