The Polytrauma/TBI Program Social Worker Coordinator is in the Physical Medical and Rehab Service. This position is located at the Manchester VA Medical Center. This is full time, 40 Hours a week.

Incumbent operates within the national Polytrauma/Traumatic Brain Injury (TBI) System of Care model as outlined in VHA Handbook 1172.01 (Polytrauma System of Care, March 21, 2013) and within Medical Center policies. The Polytrauma/TBI Program Coordinator reports directly to the SPRS Chief. Incumbent provides case management services to the Veteran population enrolled in the Polytrauma/TBI and Physical Medicine and Rehabilitation (PMR) clinics. This includes high quality case management, clinical treatment, advocacy, and coordination of linkages to other appropriate VA or community service providers/agencies as needed by the client. The Polytrauma/TBI Program Coordinator is responsible for tracking and reporting Veterans' Reintegration/Rehab Care Plans, TBI performance measures, staff tracking, and other assigned reporting required of the program. The coordinator serves as the COR overseeing the Robin Hill Farm contract. Incumbent must be able to document various workload data collection processes, including but not limited to, Decision Support System (DSS), Computerized Patient Record System (CPRS), and the VSSC National Polytrauma/TBI database. He/She must have familiarity with outcome data collection that is utilized by VHA for TBI services, however training can be provided. Duties include but are not limited to: The Incumbent has broad program administration responsibilities. The program coordinator oversees the daily operation of the program including developing and ensuring compliance with policies and procedures for program operation. The coordinator has professional responsibility for assisting the SPRS Chief in planning, developing, implementing, directing, and evaluating the Polytrauma Support Clinic Team (PSCT) program and coordinating services related to traumatic brain-injured patients and their families. The coordinator will collaborate with the Polytrauma/TBI team and support personnel in creating lean processes for patient flow from assessment, through treatment, and follow-up in primary care. The Incumbent meets with primary care, specialty care, mental health, or other staff as needed to coordinate smooth access to the Polytrauma/TBI Program. The Incumbent has comprehensive responsibilities for completing VISN mandated reports. The Incumbent will ensure that all program evaluation data are collected and entered the appropriate data base. Work Schedule: Monday-Friday, 8:00am-4:30pm. Compressed/Flexible:Available. To be discussed with Supervisor. The incumbent may be required to travel to other VA campuses and CBOC's. Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. Narrative responses to the knowledge, skills and abilities (KSAs) may be required from the selectee in order to proceed with the appointment.

Basic Requirements: BASIC REQUIREMENTS. The basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. § 7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a social worker in VHA, all applicants must meet the following: a. ENGLISH LANGUAGE PROFICIENCY: To be appointed to serve in a direct patient-care capacity in VHA, candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f). b. CITIZENSHIP: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part). c. EDUCATION: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. LICENSURE: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
