This position is for a Nurse (Community Health) at the following facilities: Uintah-Ouray Service Unit in Fort Duchesne, Utah Irene Benn Health Clinic in Moapa, NV Parker Indian Medical Center in Parker, Arizona Moapa Health Center in Moapa, NV Peach Springs Indian Health Center in Peach Springs, Arizona Supai Health Clinic in Supai, AZ Hopi Health Care Center in Polacca, Arizona Fort Yuma Service Unit in Winterhaven, California

Duties

Formulates and implements overall community health nursing programs and plans to satisfy varying patient load requirements, priorities for service and other healthcare objectives. Tracks community health status and evaluate health trends and risk factors of population groups. Collects data in a systemic organized manner and analyzes the collected assessment of data. Investigates health problems and hazards such as communicable disease and develop a recommended plan of action to address these issues. Provides health education to individuals, families and community groups in various settings. Performs community assessments to develop a plan of action; and collaborates and establishes partnerships with Tribal Community Health Representatives, Local, State and Federal systems.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree From Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure: Applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-9 and above, many positions may require experience in a specialty area of nursing.GS-9 Basic Requirements: In addition to the above requirements, applicants must meet one of the following requirements: Possession of a master's degree in professional nursing; OR Possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program and at least one full year of professional nursing experience (equivalent to the GS-7 grade level or above): Provide community health nursing services in clinics, homes, schools, and communities with minimal supervision Conduct home visits involving case findings, referral for treatment and follow-up care; Plan and conduct community health programs related to disease prevention/risk reduction. GS-11 Basic Requirements: In addition to the above requirements, applicants must meet one of the following requirements: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; OR Possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program or at least one full year of professional nursing experience (equivalent to the GS-9): provide highly specialized nursing care to high risk populations in the setting of clinics, homes, schools, and communities; develop health education materials, investigates health problems in the community to identify potential/outbreak situations-field epidemiology; leads and coordinates wellness clinics. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements by the closing date of the announcement.
