The Hudson Valley Health Care System is looking for one Medical Instrument Technologist.
Duties
Major duties and responsibilities:
-Performs a full range of ultrasound procedures and special, complicated examinations that include, but are not limited to, the abdomen, pelvis, transvaginal, chest, small parts, thyroid, and scrotum.
- Performs vascular studies such. as carotid, vertebral, renal artery, aortic aneurysm, portal-systemic shunts, etc.
- Performs equipment system check prior to use to assess acceptable performance based on established guidelines. Troubleshoots equipment in diagnostic procedures and can make minor repairs and/or adjustments. Must recognize and report major equipment malfunctions within a timely manner to service representative and imaging program manager, initiating work orders necessary to drive repairs when there are equipment problems or down time.
- Calibrates equipment following established calibration parameters making adjustment to the system as needed.
- Navigates the patient's electronic clinical record prior to exam, including, clinical diagnosis laboratory results and reviews prior ultrasound/vascular exams and other exams to prepare for and to understand existing pathology. Maintains electronic records and files of exams performed on patients.
- Explains examinations Monitors Patient physiological changes throughout the procedure keeping Radiologist informed as necessary.
- Schedules exams in a manner to optimize scanner times while maintaining flexibility for emergency cases. This requires the ability to prioritize scheduled patients as well as the ability to accommodate the emergency patient who may need immediate emergency surgery, etc
- Confers with radiologists to determine requirement of non-standard exams. Notifies them of scans requiring their immediate attention. Communicates with coworkers, hospital staff and provides successful hand off of patients after an exam is completed. Makes necessary documentation in CPRS regarding patient status and completes incident reports for all infiltrates, extravasations and other unforeseen events.
- Receives and interprets all imaging requests. Inputs pertinent radiology information into computer. Registers, requests, cancels and edits exams via the computer radiology information system (RIS). Compiles computer generated x-ray reports and distributes to appropriate clinics and physicians.- Explains examinations to be performed to the patients so that an understanding of the purpose of the study as well as any requirements of the patients are thoroughly understood.
- Responsible to explain exams to competent patients in terms the patient will understand what is to be done and the hazards, if any.
- Interviews patient obtaining pertinent medical history and precise medication usage as it pertains to the cardiac system. Explains to competent patients the rationale of procedures and instructions on how to complete the procedure.
- The imaging technologist must be certain patient identification and the requests are identical. Properly identifies patient images ensuring accuracy of patient identification, correct site, and correct side. This is essential so that mistaken identity of patient images or that imaging of an incorrect part or side does 'not occur. The technician must be certain that the patient I.D. band or ID card and the request are identical.
- Performs routine clerical procedures for the purpose of maintaining accurate imaging records so that easy recall of patient examinations may be achieved. This requires maintaining an appropriate procedure log, editing and when necessary, registering patients in the Radiology Information System (RIS).
- Gathers related information such as patient chart, imaging reports, etc., so that correlation of examinations to other studies may be made by the interpreting provider in diagnosis and recommendation or treatment for medical disorders.
- Works with the radiologist as a member of the imaging team. Must assist and anticipate the needs of the radiologists and the performance of the exam. Must be continuously on the alert for the patient's condition and intravenous site complications.
- Ability to vary tests or treatment procedures as needed to compensate for patient's individual medical conditions and body habitus.
- Performs general and vascular ultrasound examinations often abdomen, neck, chest and extremities, retroperitoneal space for the purpose of acquiring high quality examinations that assist in the diagnosis and treatment of patient medical disorders.
- Advises interpreting provider of significant abnormalities that would require their 'immediate attention and visualization while the patient is undergoing examination- Distributes completed examinations and related information to the radiologist responsible for the interpretation of the exams. This is done so that treatment based on diagnosis may be effectuated in a timely manner.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Citizenship. Citizenship of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
b. Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions.
c. Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under "Titles and Certification/Registrations".
For all specialties identified in this standard, Basic and Advanced Cardiac Life Support certifications are desirable. Training which does not result in official certification does not meet this definition.
Diagnostic Ultrasound Technicians operate diagnostic ultrasonic scanning equipment to produce cross sectional and two dimensional pictures of internal organs and body structures used to diagnose diseases and other medical conditions. Registration is offered by Cardiovascular Credentialing International (CCI) or by the American Registry of Diagnostic Medical Sonographers (ARDMS).
Grade Determinations:GS-8
(a) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. This would be experience which provided the overall knowledge, skills, and abilities to conduct diagnostic ultrasound examinations in complex and unusual cases. In addition, the candidate must demonstrate the following KSAs:

1. Ability to perform diagnostic ultrasound examination on multiple areas of the body, including, but not limited to, the abdomen, pelvis, transvaginal, chest, small parts, thyroid, breast, and scrotum.
2. Ability to perform vascular studies such as vertebral, renal artery, aortic aneurysm, portal-systemic shunts, etc.
3. Ability to assist radiologists with biopsy procedures determining the location, depth, and required needle angle for the lesion to be sampled.
4. Ability to assist in surgical procedures and fluid aspirations.
Certification. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are desirable.
Assignment. This represents the full performance level for medical instrument technician (diagnostic ultrasound). At this level the technician is required to perform a full range of procedures including special complicated examinations for which there are no standard instructions or procedures. They perform independent portable ultrasound examinations in the intensive care units, emergency room, surgery, and throughout the Medical Center when required. They also work independently on evenings, nights, and weekend tours of duty as scheduled and/or when on call

The full performance level of this vacancy is GS-9. The actual grade at which an applicant may be selected for this vacancy is GS-8.
