The West Palm Beach, FL VA Health Care System is currently recruiting for an Registered Nurse - Travel Veteran Coordinator (TVC). The TVC position may require focused assessment, critical reasoning, and astute clinical judgment in order to expedite appropriate, timely care and treatment, especially since the patient may present with complex problems or potentially life-threatening conditions and/or behavioral health emergencies.

IF YOU ARE NOT A PERMANENT VA EMPLOYEE, YOU SHOULD APPLY UNDER THE ANNOUNCEMENT CBTC-12221714-24-CB*** The Traveling Veteran Coordinator (TVC) is the point of contact for coordination of care for veterans who are relocating or have non-urgent medical needs while traveling outside of the preferred VA facility catchment area. The TVC is responsible for the coordination of care for Veterans with acute and/or chronic medical, surgical and/or mental health conditions. The TVC collaborates with ordering providers and TVCs from preferred facilities to ensure appropriateness and timeliness of care coordination. The TVC develops and coordinates the seamless transitioning of patient care throughout the VA system, with a focus on care coordination, regional/national professional collaboration, and systemwide resource utilization. The TVC provides coordinated care across the life span considering state of health, race, culture and values to Veterans, families, caregivers. TVC nursing care coordination occurs across the continuum of care in a variety of outpatient settings, which include but are not limited to clinics and Telehealth service environments. The TVC registered nurse focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating nursing and other health services, and assisting the patient to navigate the health care system. Core elements of performance include: Using the nursing process, multidisciplinary collaboration, coordination of appropriate health care services and community resources across the care continuum, active participation in unit/program level quality improvement process and initiatives as well as customer service programs. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. The TVC is a proficient user of all VA computer programs and systems related to the traveling veteran program. The TVC possesses a broad knowledge of systemwide considerations for access to care and implements a multidisciplinary approach to care coordination. The TVC maintains compliance with all national TVC practice guidelines, including those related to documentation and follow up communication between VA facilities. The TVC will advocate for the needs of traveling veterans at the unit, service, and organizational levels. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Experience with the Traveling Veteran Program PACT/Primanry Care experience Care Coordination Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The RN may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/ invasive procedures. The RN may occasionally be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The RN must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The environment's pace can vary from steady to a sometimes fast paced, requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a noisy environment.
