The position is located in Hoffman Estates Community Based Outpatient Clinic. The Physician Assistant (PA) will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the DVAHCS. The PA is responsible for the evaluation and management of a panel of patients under his/her care. The PA provides comprehensive preventive and wellness care, as well as spectrum of acute, episodic, chronic and end of life care to the Veterans.
Duties
Role and Responsibility:
The PA practices general medicine with a focus on community-based outpatient clinical care
The PA independently follows his/her own panel of general medicine patients, functioning as their primary care provider. This primary care/gatekeeper role is incumbent with a high degree of responsibility and independence.
The location of the CBOC away from the Medical Center means that the PA's assessment skills and breadth of knowledge must be superior. In addition, she must be a highly effective communicator in order to negotiate with consultants at the VAMC regarding the triaging of patients he refers. These skills are required at a level that exceeds that of VAMC-based physician assistants.
The PA uses evidence-based practice to work collaboratively with the Teamlet (Patient, RN Care Manager and Medical Support Assistant) and extended Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient) in developing the patient-driven holistic care plan for life. The PA will provide primary care, to include Women's health care, for a panel of approximately 900 patients , will participate and maintain clinical and educational activities within Primary Care/General Medicine at Hines to stay grounded and relevant in the role. Women's Health Care updated training for VA care delivery will be provided as needed. Responsible for all functions which fall under the scope of care of Primary Care.
The PA is a member of the Teamlet providing diagnosis and treatment. The PA provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The PA is prepared in advanced practice across the health continuum. This practice includes independent and interdependent decision-making and direct accountability for clinical judgment. The PA role includes participating in and use of research, development and implementation of health policy, leadership, education, case management and consultation. The PA provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
PRACTICE DIMENSION:
Apply assessment, diagnosis, outcome identification, planning, implementation, and evaluation to systems or processes at the Teamlet level to provide patient-driven holistic care.
Provide medical services to Veterans and their families/caregivers in both individual and group settings.
Emphasize health promotion and disease prevention; diagnose and manage acute, chronic and episodic illnesses.
PA practice dimensions may include but are not limited to ordering, conducting, and interpreting diagnostic and laboratory tests and prescription of pharmacologic agents and non-pharmacologic therapies.
Teach and counsel Veterans, families/caregivers and groups.
Demonstrate leadership by involving the Teamlet and larger Team in providing customized, evidence-based patient-driven care.
Design, implement and revise interdisciplinary patient-driven holistic care plan for life.
Integrate biopsychosocial concepts, cognitive skills, cultural, population and age-specific patient characteristics to provide holistic care.
Educate and encourage patient self-management.
Serve as a resource and advocate in the protection of patient and family rights.
Participate in system review to critically evaluate and assess resource utilization and safety issues with care teams and updates life plan accordingly.
Effect change through advocacy for the profession, Teamlet, extended Teamlet and the patient.
Maintain privacy and confidentiality of all patient information, including electronic, print, and conversations.
Apply skills to promote patient self-management toward patient-driven holistic care plan for life.
Provide patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.
Is proficient in fundamental management of acute and chronic medical conditions
Utilization and recognition of appropriate consultative services
Proficient in employing diagnostic and therapeutic treatment plans per the approved standard treatment guidelines.

Physician Assistant Basic Requirements
Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~
Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~
Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
Grade Determinations:
In addition to the basic requirements stated above, the following qualification criteria must be met in determining the appropriate grade assignment of candidates:
(1) Physician Assistant-I (PA-I)
(a) Experience and Education. None beyond the basic requirements. This is the entry level for PAs.
(b) Demonstrated Knowledge, Skills, and Abilities. The following list of knowledge, skills, and abilities (KSA) present the minimum skill set expected of an Entry Level PA. This practice may include, but is not limited to:
i. Ability to utilize the principles of PA practice in the care of assigned patients to provide appropriate care.
ii. Ability to perform patient history and physical.
iii. Ability to formulate a plan and provide treatment to the patient.
iv. Ability to provide patient care that is abreast of changing concepts and advancements in the profession.
(2) Physician Assistant-II (PA-II)
(a) Experience and Education. The PA at this grade is at the intermediate grade level and has had progressively increasing responsibility and experiences as indicated in the KSAs listed below. Individuals will have demonstrated the ability to analyze and solve more complex medical problems than at the entry level grade and can utilize their training in the successful practice of patient care and participate in team problem solving. Individuals demonstrate the KSAs of PA-II intermediate level by:
i. Two years of successful practice as a PA.
OR
ii. Successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency.
OR
iii. A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education.
(b) Demonstrated Knowledge, Skills, and Abilities (KSAs). PAs at this level are not required to meet each individual KSA listed below to qualify, but should be able to demonstrate, develop, or acquire those not met upon appointment for PA-II:
i. Skills to analyze and solve complex medical problems.
ii.Ability to function autonomously and make independent medical decisions.
iii. Skill to incorporate training and experience into the successful practice of patient care including telehealth medical care.
iv. Ability to work effectively in an interdisciplinary environment for improvement of services for patient care.
v. Ability to practice with colleagues and peers collaboratively.
(3) Physician Assistant-III (PA-III)
(a) Experience and Education. PAs at the PA-III grade are at the full performance level and lead and organize patient care independently, practice within multi disciplinary teams, and coordinate all aspects of patient care across the health care system. They utilize advanced clinical knowledge and judgment in decision-making and evaluating outcomes. They function as experts in clinical practice and/or in areas related to the assigned roles and responsibilities, may serve as mentors for transitioning PAs, and may provide training for health care students. Individuals can demonstrate the KSAs for the full performance level by:
i. Three years of successful practice as a PA.
OR
ii. Successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency and one year of professional practice as a PA.
OR
iii. A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education and one year of professional practice as a PA.

The full performance level of this vacancy is PA-III. The actual grade at which an applicant may be selected for this vacancy is in the range of PA-I to PA-III to .
