This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

One Full time position exist for an experienced registered nurse in the Geriatric Extended Care (GEC) Home Based Primary Care (HBPC) Program to participate as a member of the interdisciplinary team providing direct primary care to a specific group of homebound patients with a variety of chronic and terminal illnesses. The RN is responsible for Care Coordination on an assigned caseload utilizing professional nursing skills in collaboration with the interdisciplinary team members led by the licensed independent provider. Responsible for implementing the home health plan that meets each Veteran's specific need that complies with the organizational standards and federal and state regulations, and the VAMHCS policies and procedures. Participates in weekly Interdisciplinary team meetings (IDT) and in the performance and continuous quality improvement activities and initiatives. May oversees the nursing care of Veterans receiving care in community- care sponsored or contract programs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Grandfathering provision. All persons employed in VHA in this occupational series and performing the duties as described in the qualification standard on the effective date of this qualification standard are considered to have met all qualification requirements for the grade held including positive education and licensure/certification. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: Nurses who are retained under this provision and subsequently leaves the occupation, loses protected status and must meet the full VA qualification standard requirements in effect at the time of re-entry to the occupation Nurses initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification and meet all the basic requirements of this qualification standard, must maintain the required credentials as a condition of employment in the occupation Nurses who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard Graduate from a professional school of nursing accredited by one of the following at the point of graduation is required: Accreditation Commission for Education in Nursing (ACEN); or Commission on Collegiate Nursing Education (CCNE) Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: At least BSN, MSN preferred Certification in case management, geriatrics, or ambulatory care is preferred 2-3 year's experience with home care home based nursing, medical, surgical or primary care outpatient clinic Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - Diploma or associate degree in nursing and one year of specialized nursing experience equivalent to a Nurse I, Level 1 OR Bachelor's degree in nursing and no professional nursing experience. Nurse I Level III - Diploma or associate degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 2. OR Bachelor's degree in nursing and one year of professional nursing experience equivalent to Nurse I, Level 2. OR Master's degree in nursing and no professional nursing experience. OR Master's degree in a related field with a bachelor's degree in nursing with no professional nursing experience. Nurse II - Bachelor's degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 3. OR Master's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3. OR Master's degree in a related field with a bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3. OR Doctorate degree in nursing with no professional nursing experience. OR v Doctorate degree in a related field with a master's or bachelor's degree in nursing with no professional nursing experience. Nurse III - Master's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III. OR Master's degree in a related field with a bachelor's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III. OR Doctorate degree in nursing with one year of specialized nursing experience which is equivalent to Nurse II and meets all dimension requirements for Nurse III. OR Doctorate degree in a related field with a master's or bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. This position also requires the ability to lift 30 lbs. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment.

Foreign Nursing Education
  • Graduates of foreign schools of professional nursing who, possess a current, full, active and unrestricted registration, will meet the education requirement of graduation from an approved school of professional nursing to warrant appointment as a Nurse who has completed an associate degree/entry level Nursing education program
  • Credit for foreign nursing education higher than the associate degree/entry level requires a formal degree equivalency validation from a recognized degree equivalency evaluation agency accepted by VA such as the International Consultants of Delaware (ICD)
