The Columbia VA Healthcare Center is hiring an Inpatient/GEC Community Care Program Coordinator within the Physical Medicine and Rehabilitation Service. The Program Coordinator will be responsible for administering physical therapy programs to adult and geriatric patients whom are referred from all areas of the medical center and will spend at least 25% of their time working as a subject matter expert for Inpatient/GEC Community Care referrals.

Major duties include, but are not limited to:
Demonstrates a full professional and current understanding of the theories of physical therapy treatment and their proper application through which the use of therapeutic physical rehabilitation and reconditioning procedures are implemented.
Communicates with the multidisciplinary team, advising the ream of the patient's needs and progress, exchanging discharge planning information with them. This communication occurs through team meetings, written reports, and informal communication, such as personal and telephone conversations. Written communication is accomplished in a timely fashion.
Works with a high degree of independence, exercising independent professional judgement in planning and carrying out treatment programs to meet complex treatment objectives. The Incumbent exercises originality in solving problems not covered by these guides, making adaptations, and modifications of treatment plans and procedures without supervisory guidance, to meet the complex and highly varying needs of the patient, and advises all other hear care team members and students in this regard for patients with complex conditions.
Independently reviews GEC Community Care consults and answer them in a timely manner.
Documents the denial or approval of Physical Therapy services in CPRS via an E-consult and sign on the referring provider(s) per policy.
Serves as a consultant to the facility rehabilitation team regarding community care GEC processes to include but not limited to using the appropriate facility templates for extension of services, procurement of medically necessary DME, and placing consults to the appropriate service lines to prevent delay in care.
Provides site visits to contract nursing home facilities as determined by the supervisor to maintain as well as enhance communication between the VA and community partners.
Contributes to learning opportunities at the local level via conference planning, presentations at conferences, knowledge sharing, and consultation.
Serves as a point of contact providing guidance to PM&R and facility leadership on matters related to Inpatient/GEC Community Care referrals.
Possesses the ability to establish and maintain collaborative relationships within the department, facility, affiliated programs, and/or outside community partners. The Incumbent can guide the work of a multi-disciplinary team.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Physical Therapists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d).
Education and Experience: Physical Therapists must meet one of the education/experience requirements below:
Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist; OR
Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist; OR
Doctorate degree in physical therapy.
License: Individuals must hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Loss of Credential. A PT who fails to maintain the required license must be removed from the occupation, which may result in termination of employment. At the discretion of the appointing official, an employee may be reassigned to another occupation, if qualified, and if a placement opportunity exists.
Grade Determinations: GS-13 Physical Therapist (Program Coordinator)
(a) Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level. In addition, the candidate must demonstrate all the following KSAs.
(b) Demonstrated Knowledge, Skills, and Abilities
1. Ability to apply advanced knowledge of contemporary physical therapy within specialty area to provide clinical guidance, training or education for internal and external stakeholders.
2. Ability to serve as a consultant within the assigned program area for intra-agency and interagency planning and to provide service coordination to optimize program continuity, efficiency and effectiveness.
3. Knowledge of and skill in management, administration and/or education methodologies. This includes the ability to monitor and track data, utilize available resources effectively, evaluate program quality, and generate reports for local, and/or VISN and/or VACO leadership.
4. Ability to develop and coordinate treatment and/or educational programs within a concentrated field or specialty area, such as, but not limited to, amputee, spinal cord injury, chronic pain, student clinical education/residency programming, polytrauma/traumatic brain injury.
5. Ability to establish and maintain collaborative relationships within department and with other departments or affiliated programs.
6. Ability to guide the work of a multi-disciplinary team.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Preferred Experience: The understanding of The 5 Lean principles, Lean philosophy, and forms of waste. Prefer that the applicant has participated in at least a yellow belt project and/or additional process improvement projects within the facility

The full performance level of this vacancy is GS-12 . The actual grade at which an applicant may be selected for this vacancy is GS-13.
