The Clinical Pharmacist is responsible for the interpretation & processing of outpatient & inpatient medication orders.

The Clinical Pharmacist is responsible for managing multiple complex programs such as medication protocols, therapeutic drug monitoring, adverse drug reactions, medication reconciliation, medication histories and medication counseling for outpatients and inpatients in acute, subacute, and extended care units. b. The Clinical Pharmacist is responsible for the interpretation and processing of outpatient and inpatient medication orders, evaluation of drug therapy, dispensing of appropriate medication and the counseling of patients on proper use, monitoring and storage of medications. Major Duties A. Clinical 1) Participates in multidisciplinary team rounds when assigned to an inpatient unit. Provides consultative services that foster appropriate, evidence-based medication use. Manages medication protocols in accordance with approved scope of practice and prescriptive authority. Monitors patient response to therapy and provides appropriate recommendations regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration safety, efficacy and pharmacoeconomics. 2) Interviews and evaluates patients on admission and discharge and as necessary during stay when assigned to an inpatient unit or within ambulatory care settings for appropriateness of pharmacologic therapy and develops a pharmaceutical care plan for those patients receiving complex and high-risk medication regimens. 3) Evaluates drug therapy through direct patient care involvement for inpatients and outpatients, with clinical assessment and objective findings related to patient's responses to drug therapy and communicating and documenting those findings and recommendations to the appropriate providers and in appropriate records (e.g. patient's medical record). Documentation is done in a timely manner and consistent with the facilities established standards of practice. B. Distributive 1) The Clinical Pharmacist performs all duties that are considered routine of a staff pharmacist at VAEKHCS without a standard operating procedure (i.e. processing and filling medication orders). 2) Interprets and evaluates inpatient and outpatient medications orders for appropriateness of drug therapy. Orders are checked for patient allergies, drug interactions, incompatibilities and duplication. 3) Identifies, interprets and resolves prescribing and dispensing issues, including pharmacotherapeutic and formulary management. Documents interventions as necessary. C. Education 1) Provides orientation and training to new employees, including technicians, volunteers and pharmacists. 2) Serves as a preceptor for pharmacy clerkship students and pharmacy residents. 3) Maintains professional competency on drug therapies and innovations in pharmacy practice pertinent to the care provided in this health care system. D. Administrative Support/Program Management 1) Provides technical guidance and monitors pharmacy support staff assigned to their work areas. 2) Performs proper recordkeeping and maintains proper documentation when required or appropriate for all dispensing activity and patient interventions. 3) Ensures sensitive data for patient files, both printed and electronic, remain confidential. Ensures ADP systems for pharmacy practice and patient care are utilized for official duties and remain secure. E. Research 1) Designs and/or participates in drug related research studies under FDA guidelines that have undergone review and approval by the Research & Development Committee and the Investigational Research Board. Work Schedule: Primarily Monday through Friday, 8-hour-shift worked between hours of 6:30am and 5:00pm with rotating evenings, weekends, & holidays based on coverage needs of the department Telework: Not Available Virtual: This is not a virtual position. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited college or School of Pharmacy with a baccalaureate degree in Pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOFEL iBT). COPY OF TRANSCRIPTS REQUIRED Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. Copy of current license required if applicable. Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience or Education. In addition to the basic requirements listed above, applicants must have 1 year of experience equivalent to the next lower grade level OR have completed an ACPE-accredited Pharm.D. program (transcript required). In addition, pharmacists at the GS-12 level must also demonstrate the following Knowledge, Skills and Abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge or laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Inpatient/Outpatient experience and/or PGY-1 Residency Program preferred. References: VA Handbook 5005/55, Part II, Appendix G15, Licensed Pharmacist QUALIFICATION STANDARD dated June 7, 2012. This can be found in the local Human Resources Office. The full performance level of this vacancy is GS-12. Physical Requirements: Physical Requirements: heavy lifting (45 lbs. and over); reaching above shoulder; use of fingers; walking (3 hours per day); standing (6 hours per day); kneeling (2 hours per day); both legs required; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16"; far vision correctable in one eye to 20/20 and 20/40 in the other; ability to distinguish basic colors; hearing aid permitted.
