Program Management: 1) Serves as a resource to EKHCS staff, including Pharmacy leadership, clinicians and administrator regrading sterile compounding, USP 797 & 800 guidelines and related educational outreach programming. 2) On an ongoing basis reviews VA national, VISN and local initiatives and performance measure metrics to develop corrective actions and policies. 3) Responsible for the development, revision, and execution of Pharmacy sterile & non-sterile compounding as well as hazardous drug handling policies and procedures. 4) Seeks leadership and clinician input into design of projects; validates the structure and function of information management systems and recommends appropriate implementation strategies for planned activities. 5) Retrieves best practice information from model sites and implements efforts for quality management. B. Clinical Functions: 1) Directly involved in the design, implementation and monitoring of therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in assigned areas. 2) Obtains medication histories from patients and summarizes significant findings in Computerized Patient Record System (CPRS) and/or to the provider as appropriate. 3) Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindication, side effects, potential drug interactions, and appropriate therapeutic outcomes. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. 4) Evaluates drug therapy to optimize appropriateness, effectiveness and cost-benefit and communicates with multidisciplinary team members and pharmacy services. 5) Monitors for and reports drug errors, adverse drug reactions, allergies, and patient compliance issues. C. Education & Training Functions: 1) Designs and provides education based on targeted performance areas and data to targeted providers, pharmacists and clinical teams. 2) Works with clinicians to identify, develop and implement target programs. Develops and performs formal and informal education to promote these programs and gain acceptance. 3) Develops training resource materials for VASNHS staff and patients. 4) Participates in the training and in-servicing of pharmacists, pharmacy technicians 5) Effectively instructs patients and family members in the appropriate use of medications and medical devices. Able to adjust communication and fracking methods based on age and developmental considerations. Able to accurately assess and document patient comprehension. Able to refer patients to educational programs provided at the medical center by multidisciplinary teams. D. Drug Information: 1) Serves as an authority on drug usage, interactions, dosages, and compliance to medical staff, nursing staff, technicians, and students. 2) Uses appropriate references to research drug information. Has a good working knowledge of information resources, both written and electronic. 3) Possesses a working knowledge of information resources. 4) Maintains a current knowledge of therapeutics and disease management. 5) Provides timely and accurate responses to drug information inquiries from all customers. E. Performance Measures: 1) Maintains current knowledge, responsibility, and understanding of VISN 15 PBM, local and national performance measures. 2) Maintains current knowledge for access and use of VISN 15 PBM dashboards. 3) Provides and promotes cost effective pharmacy services, offering reasonable and appropriate alternatives to high cost drugs. 4) Identifies new performance measures to improve-quality care. F. Administrative Functions: 1) Attends and contributes in workshops, lectures, journal clubs, case presentations, in-service meetings, team meetings, particularly in area of specialty. 2) Understands and adheres to policies and procedures of Pharmacy Service and VISN 15; includes Joint Commission, Office of Inspector General, Drug Enforcement Agency, American Society of Health-System Pharmacists as well as other VA Pharmacy regulatory requirements. 3) Performs all routine duties of Clinical Pharmacist as outlined in Pharmacy Service Policy. G. Research: 1) Actively collaborates with EKHCS staffing the design and implementation of research protocols, and collection of data and participates in the analysis and interpretation of the results. 2) Participates in collaborative research studies involving drug therapy with other services. 3) Supervises projects as required by pharmacy residents and students. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Grade Determinations: Sterile Compounding Facility Compounding Manager, GS-0660-13 (Above Full Performance) Experience: In addition to the Basic Requirements, Pharmacists assigned to the GS-13 grade must have 1 year of experience equivalent to the next lower grade level (GS-12): Assignments: For all assignments above the full performance level, the higher the level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. The Compounding Sterile Product (CSP) Program Manager is responsible to oversee, manage and coordinate the compounding related initiatives, safety, and performance improvement within Eastern Kansas VA Health Care System. The CSP Program Manager is responsible for program management of the preparation of IV solutions, chemotherapies, hyperalimentations for administration to the veteran population. The CSP Program Manager works with the IV Lead Pharmacy technicians as well as other appropriate pharmacy personnel and management to assure all regulatory and accreditation standards are met. The CSP Program Manager provides evidence-based pharmaceutical care services. The incumbent has oversight to manages compounding on current USP 795, USP 797 and 800 policies and clinical knowledge that is consistent with policies established at the national, VISN, and local levels. Incumbent will perform data analysis to measure performance and trend changes. As such, position requires computer expertise, particularly in database programs and data analysis. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organized, manage, and control complex pharmacy programs. Develops, organizes, coordinates, and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is a reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacist assigned to this position must demonstrate KSAs as the following: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Oncology and Specialty medication admixing and/or prior authorization process. Knowledge of USP 800, USP 797, & USP 795.
