This position is located at the James A. Haley Veterans' Hospital, located on site in Tampa, FL. This position serves as program coordinator and is administratively responsible for the Medical Foster Home Program. Coordinator manages the daily operations of the program, and provides direct patient care services in the program area. Responsibilities include the daily operation of the program, developing policies and procedures for program operation and preparing reports and statistics.

Duties

This position serves as program coordinator and is administratively responsible for the Medical Foster Home Program. The incumbent reports to Supervisor, Geriatrics and Extended Care, but practices independently, exercising initiative and independent judgment in daily activities. In conjunction with the Social Work/Geriatrics and Extended Care Section Chief, the incumbent plans, organizes and directs administrative, operational, patient and personnel activities for the service. Additional Duties include but not limited to: Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Assessment, treatment, and planning for a complex population of patients which reflect the age-specific, psychosocial, and functional needs of the patient (e.g., spinal cord, geriatric and mental health population). Demonstrates knowledge of VA and community resources to meet the complex psychosocial needs of this unique population of patients and to ensure they are referred to the least restrictive environment that meets the level of care needed. Performs a wide range of services and provides technical assistance to State, local, and private agencies, and universities. Uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, State, etc.). Has knowledge of Federal and State laws and regulations that pertain to the care of vulnerable adults in community residential homes. Coordinating initial fire and safety inspections of these homes to ensure the safety of the veteran population served. Coordinates the inspection of the homes on an annual basis. Performs work related to the review, monitoring, analysis, planning and assessment of the Medical Foster Home Program. Responsible for key project activities related to the planning, development, and implementation of the project component. Handles crises and effectively uses his/her problem solving skills to assist the veteran, family and staff. Monitors homes with unannounced visits at least once per month. Communicates with entire multidisciplinary team regarding veterans contentment with home and care received in the home. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day coordination complex to administer. **Travel to outreach sites and veterans homes is required via government vehicle so a valid state driver's license is required for this position.* Preferred Experience: Clinical social work within a community area Experience with geriatric population Work Schedule: Monday through Friday, 8:00 am to 4:30 pm Telework: Available - per agency/manager policy Functional Statement #: Social Worker Program Coordinator / FS 71159F Financial Disclosure Report: Not required Additional Benefits: Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior work experience or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Applicants must meet the requirements by the closing date 02/12/2024 of this announcement. Basic Requirements: United States Citizenship: Must be US Citizen. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker Program Coordinator candidates must have the following: Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Program Coordinator social workers have experience that demonstrates possession of advanced practice skills and judgment. Social workers are experts in their specialized area of practice. They may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Training or additional certs must be submitted with application. AND Licensure/Certification: At this grade level, social workers must be licensed or certified by a state at the advanced independent practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Incumbent must have a valid driver's license and pass Occupational Health screening as position will require driving a government vehicle. Incumbent must be able to climb stairs and walk short distances. Incumbent must be able to lift 25 pounds.
