The incumbent provides care management in the PRC Department at the Lawton Service Unit. He/she is a crucial member of the PRC team with active participation of other clinic care staff; which may include: doctor, physician's assistant, advanced practice nurse, registered nurse, licensed practical nurse, pharmacist, public health nurse. community health representative as well as patients. This position under the supervision of the Purchased Reffered Care (PRC) Manager or designee.

Duties

Collaborate with primary care provide. clinic nurses. public health nursing, DM case management, social services, and con1raeted vendors to ensure that the patient's care needs are completed in a timely and cost effective manner to maximi7.e each visit. Serves as primary contact/clinical liaison between referral hospitals. treatment facilities and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established plan of care. Contacts patients for follow-up diagnostic tests. referrals, appointments, or other medical issues as appropriate in collaboration with the provider. Provides direct nursing care for patients presenting with "STAT" referrals.

BASIC REQUIREMENT(S): A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Must submit copy of transcript. Applications will be considered incomplete without proof. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. AND Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, you must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Experience in providing care coordination; collaborating with multidisciplinary health team members for patient care planning; servicing as a liaison between patients and healthcare providers and between healthcare facilities evaluating and providing patient and family health education needs; facilitating transitions in patient care, and working with healthcare referrals that involve eligibility requirements such as the HIS Purchased and Referred Care program or Medicaid. Working knowledge of the Catastrophic Emergency Health Fund (CHEF) program, including guidelines and procedures. Working knowledge of the Federal Medical Care Recovery Act (FMCRA) requirements and PRC responsibilities. Ability to identify terminal illness (TERI) patient descriptors and guidelines. OR Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field. Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP). You must meet all qualification requirements within 30 days of the closing date of the announcement.
