The incumbent assists and prepares Veterans for ambulatory care visits, communicates with other members of the healthcare team regarding patient concerns and personal observations, may provide cross coverage for other areas as assigned by manager. Travel may also be required within the Health Care System. Demonstrates the ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.

Duties

Functions/Scope of Assigned Duties: Overview Serve as a responsible member of the nursing team and appropriately interact with Veterans, family members, clinicians, and supportive personnel related to patient care activities and outcomes, adhering to acceptable, established customer service standards as outlined below. The work involves the performance of a full range of functions using guides, policies, and general supervision of an RN or LIP. May work under the direction of a senior LPN GS 7. The work involves a variety of Veterans of diverse cultural, spiritual, and education backgrounds. The work directly affects the overall well-being of Veterans served. The work involves scheduled appointments, walk-ins, and telephonic or virtual interactions. The LPN is expected to have a broad working knowledge of practical nursing procedures; however, completion of more complex practices or procedures may be subject to review by a more seasoned LPN, RN, or LIP. Equipment for ambulatory care includes items such as non-critical reusable medical equipment (RME) used for obtaining vital signs, electrocardiograms, telehealth carts and cameras, and other tools for obtaining data and information about Veterans visits. Communication (Critical Element) Recognizes that illness and/or hospitalization may cause fear and anxiety which patients may express in a variety of behaviors and is able to respond in an appropriate manner to those patients. Is able to anticipate which circumstances or populations may increase the likelihood of a fear or anxiety response and intervene appropriately and promptly. - Identifies age-specific or population-specific learning needs and provides patient education materials and documents teaching in the medical record. Teaching may include pre-and post-op care, diabetic self-care, wound/stoma care, activities of daily living, self-administration of medication, common disease specific education, health coaching, injection training, blood glucose checks, dietary concerns, tobacco cessation, and additional procedures as directed by RN or LIP. - In working with Veterans, family, and/or staff, there is consistent evidence of a professional, caring approach in listening, educating, and performing procedures. This includes introducing self, addressing the patient appropriately, and explaining procedures using guidelines. - Promotes team cohesion, participates in unit activities to enhance positive work environment, and contributes to performance improvement activities. This includes responding to view alerts, secure messages, and clinic protocols and procedures. - Demonstrates increased personal commitment to the job, including mentoring less experienced LPN, NA, and HT personnel. - Ensures all members of the healthcare team are aware of patient concerns, changes in health status, or patient needs. Seeks guidance for complex or infrequent care needs. - May assist with orientation of newly hired LPN/LVN employees, including those at the same, lower, or higher grade. - Ensures documentation in the medical record reflects conversation with Veterans, families, and other members of the healthcare team, including actions taken, communication that has occurred, and follow up as needed. - Demonstrates INTEGRITY in all interactions by acting with high moral principle; adhering to the highest professional standards and maintaining the trust and confidence of all with whom the LPN engages. - Demonstrates COMMITMENT by working diligently to serve Veterans, employees, volunteers, and other beneficiaries and fulfilling the LPN's individual responsibilities and organizational responsibilities. - Acts as an ADVOCATE when self-determination is questioned, fully considering, and appropriately advancing the interests of the customer and other beneficiaries. - Treats all customers with dignity and RESPECT. Listens to and honors the customer's perspectives and choices. Customer's knowledge, values, beliefs, and cultural backgrounds are incorporated into the planning and delivery of care in a non-judgmental, non-discriminatory manner (Patient and Family Centered Care). Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Verification can be obtained from the State approving agency [or] the National League for Nursing [Accrediting Commission], 61 Broadway, 33rd Floor, New York, NY 10006. [Additional information may also be obtained from the NLNAC Website. Exceptions include: Licensure. Full, active, current and unrestricted licensure as a [ ] licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements stated in section A, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Preferred Experience: One year of clinical experience in Primary Care. References: VA HANDBOOK 5005/3 PART II APPENDIX G13 The full performance level of this vacancy is GS-6.
