The LPN possesses the knowledge and ability to appropriately carry out assigned patient care based on resident's condition. The incumbent will demonstrate skill in handling a high volume of residents with chronic diseases (e.g. hypertension, diabetes, cardiac, and pulmonary disease), as well as acute episodes of illness. Main duties, skills, and abilities required, but are not limited to: Must be able to utilize written and verbal communication skills with supervisors and other staff. Initiating, performing, and completing assigned duties in providing care to variable resident populations Appropriately carrying out assigned resident care based on the resident's conditions, using judgements in selecting the appropriate order and sequence of procedure and treatments; accurately recognizing, reporting, and recording relevant resident information. Observing, identifying, and responding to the resident's needs for care in the home, including medication, equipment assisted care, and resident/family education. In organizing and delivering care, the LPN recognizes and considers emotional, spiritual, cultural, socio-economic, and age-related factors. Observing for physical and/or emotional changes in resident's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviating from normal to RN or MD/NP. Recognizing urgent or emergent resident care situations, seeking assistance of the RN and/or MD/NP, and initiating appropriate emergency interventions, as directed. Understanding of human behavior, resident motivations, and reactions to situations, and appropriately utilizing this knowledge to work effectively with residents, family members, and other staff in all resident care settings. Establishing constructive relationship with individual residents and their caregivers to elicit feeling and attitudes and to promote positive relationships, communication and socialization skills. Performing procedures that include venipuncture, foley catheter change and care, peg tube care and wound care. Assessing the home environment for resident safety and recommends a plan of action to the resident. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing Accrediting Commission (NLNAC) at the time the program was completed by the applicant.
