VA San Diego Health Care is looking for a Registered Nurse with clinical experience with all manners of wounds within the Scope and Standards of a Certified Wound Specialist.

Duties

VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Wound Care Specialist RN assumes the following major duties and responsibilities: Specialize in wound care practice. Expertise in the prevention, identification and treatment of wounds. Provides expert hands-on care for individual patients with acute or chronic wounds (e.g., advanced treatment modalities within scope) Serves as a consultant to provide potential solutions for complex clinical cases to improve patient care and outcomes. Uses judgment & critical thinking skills to assess and identify outcomes; develop & implement an individualized care plan; and evaluate care of the patient with an acute or chronic wound or other alteration in skin integrity. Communicates effectively with the patient care team to mitigate potential causes of alterations in skin integrity. Uses evidence-based assessment techniques, tools, available data and information to identify problems and needs of the patient with an acute or chronic wound. Collects pertinent patient data using a systemic process to identify and address the patient's needs. Synthesizes & prioritizes assessment data to provide focused care for the wound care patient. Uses evidence-based knowledge and research findings to guide practice and develop strategies/interventions managing care to achieve appropriate goals and outcomes for wound care patients. Conducts an ongoing evaluation of the goals and outcomes for management of the patient with an acute or chronic wound; and uses assessment data to revise/modify the diagnosis, outcomes, plans, and strategies as warranted. Engages the patient with a wound in self-care to maximize independence & quality of life. Articulates the role and responsibilities of the WOC nurse to team members. Leads inter-professional teams to communicate, collaborate, and consult effectively. Coordinates care for the prevention and management of complications. Develops and implements evidence-based educational programs for patients, staff, and providers. Evaluates and/or revises processes, policies, procedures, and protocols/guidelines for care of patients with wounds. Provides leadership in the design of quality improvement initiatives to optimize outcomes for patients with wounds. Advocates for patients with wounds to ensure the availability and access to specialty care services, resources, and supplies. Facilitates/coordinates systems and community resources to enhance care of patients with wounds across the continuum of care. Engages consumer alliance & advocacy groups in health teaching and health promotion activities for patients. Collects and reports data relevant to the prevention and management of wounds at regular intervals. Monitors, evaluates and acts upon the data for prevalence and incidence of wounds and skin care patients utilizing tools within the VSSC Database, IPEC and local metrics for quality reports. Identifies programmatic steps to reduce risk and incidence of pressure injuries/wounds in all populations served. Collaborates with the ordering provider to recommend topical medications, medicated dressings and other supplies indicated for the treatment of pressure ulcers and complex wounds based on published National Pressure Injury Advisory Panel (NPIAP) guidelines and established facility clinical protocols. Applies knowledge from advanced preparation, current research, and evidence when making clinical decisions to achieve optimal outcomes for the patient with wounds. Uses data and theory-driven approaches to effect organizational or system changes to improve practice & outcomes for patients with wounds and determines efficacy or need for revision. Evaluates and Implements appropriate tools, instruments, and services for diverse populations who need care for wounds. Designs quality and implements process improvement studies, research initiatives, and other programs to improve health care and outcomes for patients with wounds in diverse settings. Other duties as assigned. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: 2 years of RN experience in wound ostomy care. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to wear protective clothing in isolation situations, operative/invasive procedures, or home settings.
