This Social Worker (Program Coordinator) is located at the H. John Heinz III Department of Veteran Affairs Medical Center. The incumbent functions as a Patient Aligned Care Team (PACT) Social Work Coordinator /LGBTQ+ Veteran Care Coordinator assigned to the Comprehensive Social Services SL/Social Work. They will serve in a dual role with .5 of duties/time allocated to PACT Social Work Coordinator duties and .5 of duties/time allocated to the role of LGBTQ+ Veteran Care Coordinator.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU PACT Social Work Program Coordinator Duties: The Patient Aligned Care Team (PACT) Social Worker serves as a member of the PACT team providing a variety of clinical interventions to Veterans in the primary care setting. Interventions include evaluation and comprehensive assessment (change determinate to drivers) of core social determinant of health (SDOH) domains including access to care, psychological status, functional status, economics, social support, and housing. Veterans may be referred to the social worker by a PACT team member, providers outside of PACT, self-referral, community referral or by family members, caregivers or loved ones. PACT social workers assist with care coordination and access to care for Veterans, including those identified as having high risk factors or at high risk for medical or psychological challenges. The scope of services will vary and depend upon the nature of identified issues for which the Veteran has been referred. The social worker will apply the VA PACT Social Work Practice Model endorsed by the National Social Work Program. The social worker will provide all services in a timely fashion and will modify the approach to the needs presented. LGBTQ+ Veteran Care Coordinator Duties: The LGBTQ+ Veteran Care Coordinator has a responsibility to provide direct services and care coordination for VAPHS Veterans who identify as Lesbian, Gay, Bisexual, Transgender, Queer or a related identity. The LGBTQ+ Veteran Care Coordinator (VCC) helps to ensure that culturally competent LGBTQ+ clinical services are provided within VAPHS consistent with Veterans Health Administration (VHA) directives The LGBTQ+ VCC coordinates a variety of activities including outreach, interdisciplinary activities, collaborates with the Patient Advocate, Equal Employment Office and VA medical facility leadership in responding to compliments, complaints, inquiries and recommendations from various stakeholders, including staff, patients, caregivers, congressional inquiries, White House Hotline, and others about LGBTQ+ care at the VA medical facility. This Social Worker conducts training for staff, students and community providers regarding LGBTQ+ issues and will provide education, information and liaison with resources for Veterans in the community. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring Knowledge and ability to write policies, procedures, and/or practice guidelines for the program Ability to supervise multidisciplinary staff assigned to the program Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s) Ability to provide training, orientation, and guidance within clinical practice Preferred Experience: ADD PREFERRED EXPERIENCE HERE References: VA HANDBOOK 5005, PART II, APPENDIX G39 The full performance level of this vacancy is GS-12. Physical Requirements: Traveling throughout the medical center and the community as required, performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files.
