The primary purpose of this position is to serve as a clinical case manager to provide timely, quality, comprehensive, cost effective healthcare and human performance resource utilization as well as care support for Department of Defense (DoD) beneficiaries with complex care needs.

Duties

Evaluates clinical case management activities and conducts ongoing monitoring and evaluation of the Clinical Case Management Program using objective criteria. Develops, implements, and monitors the Special Warfare Human Performance Support Group's (SWHPSG) clinical case management program for inpatient and/or ambulatory care settings, in accordance with DoD and TRICARE Management Activity (TMA) guidelines. Provides robust and comprehensive clinical case management, care coordination, and discharge/disposition services. Collaborates/coordinates with non-Medical Treatment Facility (MTF) and community healthcare facilities and services in support of care continuity.

  • Social Worker: At a minimum, qualification requirements stipulated by the US Office of Personnel Management (OPM) for the 0185 occupational series must be met. Selective Placement Factor: A current license from a U.S. jurisdiction and (CONT'D BELOW)
  • (CONT'D FROM ABOVE) be a Licensed Master Social Worker (LMSW). Twelve months of recent experience in social work/health care management for adults, children, families, seniors, or groups is desired. (CONT'D BELOW)
  • (CONT'D) Certification in Clinical Case Management for social workers from the Commission for Case Manager Certification and/or Certified Advanced Social Worker Case Manager according to the National Association of Social Workers is highly desirable.
  • Nurse (Case Manager): At a minimum, qualification requirements stipulated by the US Office of Personnel Management (OPM) for the 0610 occupational series must be met. Must maintain a valid, unrestricted, and current (CONT'D BELOW)
  • (CONT'D FROM ABOVE) registered nurse license (with no limitations, stipulations, or pending adverse actions) in a state or territory of the United States or the District of Columbia. (CONT'D BELOW)
  • (CONT'D FROM ABOVE) Twelve months of recent experience in nursing/health care management for adults, children, families, seniors, or groups is desired. Certification in Clinical Case Management for registered nurses from (CONT'D BELOW)
  • (CONT'D FROM ABOVE) Commission for Case Manager Certification and/or American Nurse Credentialing Center is highly desirable.
  • Must possess and maintain current Basic Life Support (BLS) certification

Qualifications

CONDITIONS OF EMPLOYMENT (CONTINUED): (CONT'D FROM ABOVE) Employee must pass initial and periodic short notice drug testing. Illegal drug use by employees in sensitive positions presents a clear threat to the mission of the Air Force, national security, and public safety. Complete all training requirements designated by facility, supervisor, or accrediting agencies; participates in continuing education programs to update and/or maintain license. May be assigned to MTF disaster team; may be required to participate in applicable team training/education and exercises during normal duty hours. In event of an actual disaster, may be recalled and required to respond to team assignment. This is a Mission-Essential position performing a Mission-Critical-Function which must continue uninterrupted after the occurrence of an emergency and continued through full resumption of all functions. Knowledge of medical facility organization, mission, purpose, philosophy, and objectives as well as those of the Case Management Society of America (CMSA), American Nursing Association (ANA) for nurses, National Association of Social Workers (NASW) for social workers, The Joint Commission (TJC), Accreditation Association for Ambulatory Health Care (AAAHC), Health Service Inspection (HSI) and appropriate agencies not listed to provide professional consultation to others in the MTF. Skill in oral and written communication to articulate medical requirements to professional and administrative staff and to present briefings to providers and beneficiaries. Ability to analyze, evaluate, and update treatment plans and their relationship/appropriateness to diagnoses and to make revisions as appropriate. In order to qualify, you must meet the specialized experience requirements described in the Office of Personnel Management (OPM) Qualification Standards for General Schedule Positions, Nursing Series 0610 + IOR. BASIC REQUIREMENT OR INDIVIDUAL OCCUPATIONAL REQUIREMENT: EDUCATION: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. NOTE: You must submit a copy of your transcripts. Degrees from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schoolsis required for individuals who graduated from foreign nursing schools. NOTE: You must submit a copy of your transcripts. LICENSURE: Applicants must have passed the National Council Licensure Examination. In addition, you must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. NOTE: You must submit a copy of transcripts and nursing licensure. Note: Employees currently assigned to positions in this occupational series as of September 2017 will be considered to have met the basic requirements for the position occupied. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: SPECIALIZED EXPERIENCE: Applicants must have at least one (1) year of specialized experience at the next lower grade GS-09, or equivalent in other pay systems. Examples of specialized experience includes extensive professional knowledge and skills of advanced concepts, principles, and practices to provide clinical case management, care coordination, and discharge/disposition planning; provide professional advice/recommendations to medical clinics and physicians in providing the best possible care to patients in the most economical manner; and exercise independent professional judgment to help resolve administrative issues. OR EDUCATION: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; OR at least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. NOTE: You must submit a copy of your transcripts and nursing licensure. FEDERAL TIME-IN-GRADE (TIG) REQUIREMENT FOR GENERAL SCHEDULE (GS) POSITIONS: Merit promotion applicants must meet applicable time-in-grade requirements to be considered eligible. One year at the GS-09 level is required to meet the time-in-grade requirements for the GS-11 level. TIG applies if you are in a current GS position or held a GS position within the previous 52 weeks. NOTE: Applicants applying as VEOA candidates who are current GS civil service employees or are prior GS civil service employees within the past 52 weeks must also meet time-in-grade requirements. KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Your qualifications will be evaluated on the basis of your level of knowledge, skills, abilities and/or competencies in the following areas: Knowledge of professional clinical case management functions: a) Assessment: Identification of patients for clinical case management; comprehensive collection of patient information and medical status; and continued evaluation of an established plan of care; b) Planning: Collaboration with the patient, family/caregiver, primary provider, and other members of the health care team for developing an effective plan of care; c) Facilitation: Care coordination and communication among all involved parties; and d) Advocacy: Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received. Knowledge of medical facility organization, mission, purpose, philosophy, and objectives as well as those of the CMSA, ANA for nurses, NASW for social workers, TJC, Accreditation AAAHC, HSI and Health Insurance Portability and Accountability Act (HIPAA). Knowledge of Department of Defense (DoD), Federal, State, non-profit Healthcare systems, and organizations, to include DoD Tri-Service Military Healthcare Systems, Defense Health Agency, TRICARE purchased care system, Medicare, Medicaid, and the Department of Veterans Affairs, along with basic knowledge of coding principals and practices. Knowledge of other support agencies, such as resourcing via USSOCOM Care Coalition, Wounded Warrior Project, Center for the Intrepid, and Air Force Wounded Warrior Program (AFW2). Skill in analyzing data defining health care needs of a defined population using standardized assessment tools and operating computer programs and databases such as Excel, Access, Word, etc. Ability to apply critical thinking skills and expertise in resolving complicated healthcare, social, interpersonal and financial patient situations. Ability to effectively communicate orally and in writing with a diverse group of individuals. PART-TIME OR UNPAID EXPERIENCE: Credit will be given for appropriate unpaid and or part-time work. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. VOLUNTEER WORK EXPERIENCE: Refers to paid and unpaid experience, including volunteer work done through National Service Programs (i.e., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student and social). Volunteer work helps build critical competencies, knowledge and skills that can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
