The RN - Referral Coordination Team (RCT) provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling, provision of care supportive to or restorative of life and well-being. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The Registered Nurse assigned to the Referral Coordination Team (RCT) will promote quality care characterized by substantial and continuous responsibility for population groups or integrated programs that cross service and/or discipline lines and influences organizational mission and health care. Care will be provided in the restrictive, most cost effective manner using clinical practice guidelines and performance measures. The Referral Coordination Team Registered Nurse is a specialty trained nurse who supports a variety of roles for the Medical Center. The Referral Coordination Team Registered Nurse improves timely access to care and empowers veterans to make more informed care decisions, ensure Veterans receive their chosen modality and location of care by working in a multidisciplinary team to coordinate care either within the VA or the community. Specific duties include, but are not limited to: 1. Communicates with Veterans on their health care options including local VA, other VA Medical Centers and community based eligibility. 2. Works closely with other VA Medical Centers, contracted facilities and community facilities to remove obstacles that would impede the Veteran from utilizing the services of other VA medical centers. 3. Outpatient specialty clinic care coordination and patient flow management, including appointment management. 4. Collaborates with clinic staff to ensure patients are given timely appointments and expedite new patient consults. 5. Reviews internal and/or community care referrals for medical necessity of requested services using clinical judgement and refers cases to specialty care DOAs or RCF LIP as appropriate. 6. Educate providers on medical management processes. 7. Redirection of VHASTLHCS enrollees into VHA network. 8. Triages and applies a collaborative team approach in identifying, analyzing, optimizing utilization of VHA/VHASTLHCS resources. 9. Collaborates referral services and consultant services to develop care coordination agreements (service agreements). 10. Educates staff on care modality options to include tele-health or e-consults. 11. Data mining to include, but not limited to, referrals patterns including discontinuation or cancellations patterns. 12. Provide clinical knowledge and act as a clinical resource to non-clinical multidisciplinary staff. 13. The RCRN assumes essential personnel status during emergencies . Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: BSN/MSN Previous Referral Coordinator/Case Management Experience Acute Care Inpatient Experience Advanced computer skills (Operating systems, data analysis, spreadsheets, reports, Microsoft Word/Excel/PowerPoint/Power BI) Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The population of male and female Veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. VASTLHCS is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with visits to the Emergency Department and admissions for medical co-morbidities. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The physical requirements of this position include the following tasks which may be performed intermittently: lifting of up to 50 lbs. (3 hours), sitting (2 hours), standing (4 hours), walking (8 hours), climbing (1 hour), kneeling (3 hours), bending/stooping (3 hours), twisting (2 hours), pushing/pulling (6 hours), simple grasping (8 hours), fine manipulation (8 hours), reaching above the shoulder (4 hours), and operating machinery (8 hours). This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
