The Registered Nurse is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. The RN is responsible for providing clinical leadership and expertise on community care processes and regulations. The RN will serve as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community.
Duties
The VA Community Care program of Administrative Medicine Service (AMS) encompasses various care processes functions. The care processes are: Transfer Hospital Notification RN, Consult Referral RN, Utilization, and Traveling Veteran Coordinator RN. The RN is responsible for understanding all programs and maintaining his/her expertise in the assigned area of care. The RN reports directly to and is accountable to the Nurse Manager, VA Community Care program in AMS. The RN works collaboratively with administrative staff and has no direct supervisory functions. The nurse is responsible for facilitating the fulfillment of the SAVAHCS Mission, Vision and Values and meeting AMS goals and performance measures.
Transfer Nurse (TR RN): The incumbent is responsible and accountable for the facilitation of acute care patient transfers and admissions within and outside the SAVAHCS. This includes the transfer of acute patients for the purpose of receiving specialized services and care from the transferring facilities to the receiving facilities. The TR RN ensures the appropriate communication of patient needs, clinical information (automated or manual), and the logistics of moving the patient safely between medical facilities. The incumbent monitors and resolves all quality of care issues related to the transfer or referral of acute care patients. The TR RN ensures the transfer process is carried out appropriately and timely and that maximum safety has been provided for the patient. In addition, the TR RN ensures that the transfer process is in compliance with all standards, laws and medical center policies and procedures.
Consult Nurse (Consult RN): The Consult RN is responsible for the clinical review of community care referral consults. This includes facilitating the consult process between providers to coordinate the appropriate care / services requested for the patient. The consult RN collaborates with the VACC staff to ensure timeliness and appropriateness of care / services being provided. The incumbent will review community provider documentation for assessment and/or treatment plan information, coordinate the episode of care, and complete the consult requests. The RN will monitor and track VACC consult activities as needed. The consult RN will ensure timeliness, efficiency, appropriateness, and cost effectiveness of services rendered.
Traveling Veteran Coordinator (TVC) RN The TVC RN executes position responsibilities that demonstrates leadership, experience, and creative approaches to management of complex patient care. The TVC RN is responsible for and facilitates care services for veterans traveling outside their home station requiring new and continued care and services. The RN collaborates with home station TVC RNs to meet veterans' ongoing needs and goals for optimal care outcomes. The TVC RN acts as a liaison for veterans relocating from one VA to another in order to maintain continuity of care and services.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: Heavy lifting (45 lbs and over); Heavy carrying (45 lbs and over); Straight pulling and pushing; reaching above shoulder; use of fingers; both hands required; walking and standing up to 8 hours; repeated bending; Ability for rapid mental and muscular coordination simultaneously; hearing aid permitted.
