About the Position: Single office is in the Primary Care Clinic as a Behavioral Health Consultant, 0700-1600, Monday through Friday at Kenner Army Health Clinic, Fort Gregg-Adams, VA This is a Direct Hire Solicitation

Duties

Implements psychoeducational classes to target high frequency medical and/or BH diagnoses found in the primary care clinic. Provides primary care providers and staff regular and concise feedback on the status of referred patients and recommendations for team-based care. Uses a biopsychosocial approach to implement focused, evidence-based assessment and interventions, including behavioral, cognitive, and motivational approaches that fit the fast pace of primary care. Efficiently and accurately documents information in the electronic health record during patient encounters. Maintains close collaborative relationship with Department of Behavioral Health, participates in peer reviews, clinical case coordination, and ensuring patient safety and continuity of care Practices the PCBH model of consultative service delivery in a primary care setting, completing 8 to 12+ encounters per day in appointments no longer than 30 minutes. Focuses on functional impact and symptom reduction in patients presenting with Behavioral Health and medical conditions.

Requirements

• Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation. A Childcare National Agency Check with Inquiries (CNACI) is required.
• This position has specific initial background investigation and periodic re-verification requirements since its duties require interaction with children and youth under the age of eighteen (18) years of age on a recurring basis.
• Unrestricted license or registration from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
• This position has been designated as a Testing Designated Position and is subject to a pre-employment screening and random testing thereafter.
• This position requires the completion of a pre-employment Physical Examination.
• This position has a Temporary Duty (TDY) or business travel requirement of 5% of the time.
• This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination. A candidate selected to this position will be required to sign a statement consenting to annual seasonal influenza vaccination.
• Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series.
• This position is subject to screening and update of required immunizations unless exempted for medical or administrative reasons in accordance with Army Regulation 40-562, Chapters 3 and 4.
• This position is inclement weather essential. This means that you must be willing to make every attempt to report for duty on time and/or remain on duty during severe weather conditions.
• This position has been designated "Mission Essential". In the event of severe weather conditions or other such emergency type situations the incumbent is required to report to work or remain at work as scheduled to support mission operations.
• Must be able to obtain and maintain Basic Life Support (BLS) Training and Certifications from the American Heart Association and the American Red Cross within 6 months of hire.

Basic Requirements for Social Worker (Clinical): Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education. In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below: Specialized Experience: One year of specialized experience which includes conducting comprehensive psychosocial evaluations or assessments on patients; and providing group or individual therapy to patients; and developing treatment plans for patients. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11). Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted. Licensure: A current, active, valid, unrestricted clinical license to practice Social Work independently, offered by a U.S. State, District of Columbia, commonwealth, territory or jurisdiction.
