Inpatient Psychiatric Mental Health Treatment Coordinator position, located in Nursing/Mental Health Service. The Psychiatric Mental Health Treatment Coordinator/Patient Care Coordinator serves as a clinical expert, demonstrating leadership, experience, and creative approaches to management of complex patient needs and coordinates services for veterans across the continuum of care. This position reports to a Nurse Manager.

Duties

Inpatient Psychiatric Mental Health Treatment Coordinatorreviews the patient plan with the team and facilitates goals that were established, performs case managerial duties to the inpatient psychiatric unit: screening and coordinating discharge needs and care coordination activities. - Screens and monitors veterans, provides education and support, influences adherence to guidelines by providing admission and discharge assessments and collaboratively makes appropriate care decisions in consultation with the Interdisciplinary Team in both inpatient and outpatient areas. - Demonstrates leadership in facilitating patient flow through Edward Hines Jr. Hospital utilizing a multidisciplinary team approach. - Notifies the patient flow team of anticipated and planned discharges for the day. Communicates barriers to discharge to the care team and facilitates resolutions. - Demonstrates advanced clinical expertise in several functions, including but not limited to patient admission screening in the Emergency Room departments and inpatient areas, monitoring high-risk patients, implementing activities towards health promotion and prevention management of acute and chronic illness states, assisting the Veterans in attaining optimal levels of functioning through rehabilitation, and providing supportive measures for the patient and their significant others. - Collaborates with the Hines' Case Managers, Utilization Management staff, and Patient and Transfer care coordinators, Mental Health Outpatient Interdisciplinary Team, Mental Health Outpatient Program Managers, to ensure patient optimal discharge outcomes. The incumbent is responsible for the following: Collaborating with services internal and external to the VA to facilitate care transition to meet the patients' needs effectively Provides patient and family health education on self-management, prevention, and wellness based on the patient's goals Demonstrates leadership by advocating for patients and being a team player to colleagues as they continue to enhance their and the team's professional growth, development, and practice Demonstrates integration of nursing excellence standards, resulting in improved outcomes at the team level and beyond. Communicates verbally and in writing with internal and external stakeholders, resulting in evidence of robust Collaboratory nursing system redesign, utilizes and implements LEAN Principles in change/project management at the team level and beyond. Understands and uses leading and lagging measures and other quality improvement data, and participates in strategic planning and alignment of goals with service level, facility level, and national level. The Psychiatric Mental Health Treatment Coordinator functions and duties include the following: - Decrease hospital readmissions. - Decrease hospital Length of Stay (LOS) to meet InterQual guidelines. - Anticipate barriers to discharge and intervene. - Work closely with physician, nurses, social workers, and wide range of medical and non medical professionals. - Make provisions for current and future needs of the veteran. - Serve as a patient, family, insurance, and facility advocate. - Facilitates the achievement of veteran wellness and autonomy through advocacy, assessment, planning, communication, education, resource management and service facilitation. - Use the nursing process to assess, plan, implement and evaluate care and use of resources. - Participate in patient, family, caregiver ethics committee meeting regarding goals of care. - Document Interdisciplinary plan of care. - Education needs of the Veteran is completed in collaboration with the primary nurse using the Teach back method. - Discharge planning with the initial face-face patient assessment during admission; Ensuring that patient has a safe plan for discharge, that follow-up appointments are set prior to discharge, and that 2Day post-discharge phone call follow-up is completed. Notifying the team with issues/findings. -Duties as assigned with Case Management role. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Bachelor of Science Degree in Nursing (BSN) 2 to 4 Years RN Experience RN Experience in: Mental Health; Emergency Room; and Outpatient Areas Current (Not expired) Basic Life Saver Certification (BLS) Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: This position requires potentially long periods of continued walking, standing, and sitting. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must comply with any Employee Health requirements as a condition of employment. In the performance of duties, the incumbent may be required to drive and/or ride in GSA-vehicles.
