The Captain James A. Lovell Federal Health Care Center (FHCC) is seeking a Case Manager/Transplant Coordinator Registered Nurse (RN) for Medicine Services. This position is assigned to the Office of Chief Nurse Executive at the FHCC in North Chicago, Illinois.

The Case Manager / Transplant Coordinator RN works directly with all internal patient's within FHCC to provide case management services, resources and education. The Case Manager / Transplant Coordinator RN will collaboratively work with the interdisciplinary team members to provide timely, quality care and to coordinate the patient discharge plan. The Case Manager / Transplant Coordinator RN will coordinate and manage transplant patient referrals, collaborate with VA transplant centers and provide support and instructions to the transplant patients in the evaluation and subsequent follow-up phase of the transplant process. The Case Manager / Transplant Coordinator RN will provide administrative support to ensure optimal patient care within the transplant program.
The Case Manager / Transplant Coordinator RN executes position responsibilities that demonstrate leadership, experience, and creative approaches to promote and maintain health and prevention of illness and injury based upon the nursing process, which includes:
1) Assess patient case management needs;
2) Participate in and lead team rounds and meetings to provide efficient care management of patients;
5) Collaborate with patient and family members to facilitate patient care, discharge planning and patient education;
6) Collaborates with internal and external interdisciplinary team members to formulate and implement a comprehensive plan of care; 7) Coordinate patients care and appointments through their Interfacility Consults, Tricare and Community Care Consults;
8) Collaborates with PACT teams to make certain a primary care provider is assigned with follow-up appointment;
9) Coordinates required home care services;
10) Assist patients with completion of Advance Directives and Living Wills;
12) Coordinates transportation for eligible patients;
13) Coordinate with referring physician to manage FHCC transplant referrals, screen patient to ensure criterion are met for the VA Transplant Program.
The Case Manager / Transplant Coordinator RN referral transplant duties include but are not limited to:
1) Contacting, arranging and coordinating medical tests, examinations, consults and appointments to avoid delays in care; provide age appropriate education to patient/family members/caregivers regarding the transplant process, the submission of referrals to the VA Transplant Program TRACER Share Point, and management of subsequent follow-up visits as needed for post-transplant care;
2) Serves as a liaison between FHCC and VA transplant centers to ensure referrals are completed in a timely manner; and
3) Provides documentation to meet transplant regulatory requirements, as needed.
Other duties may be assigned in support of the program.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
Have at least a Baccalaureate Degree in Nursing or; MSN/MN or Master's Degree in a related healthcare field;
Have at least 2 (two) years of increasingly complex RN nursing practice; case management experience
Have/maintain current certification in Basic Life Support (BLS);
Be acquainted with acute medical, psychiatric and behavioral health clinical treatment modalities;
Demonstrated ability in data tracking, management, analysis results;
Have certification in Case Management (CCM or RN-BC)
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: To perform in a fully successful manner, the individual must have the physical ability to perform job-related duties which require the following: light lifting (under 15 pounds); light carrying (under 15 pounds); intermittent bending, transferring, stooping, stretching (1 to 2 hours); reaching above shoulder; use of fingers; both hands required; both legs required; walking (up to 1 hour); standing (up to 1 hour); prolonged sitting (up to 4 hours); near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; and hearing (aid permitted). Individual must be able to perform the following administrative duties, word processing, computer keyboard typing, entering and retrieving database information, computer screen viewing, answering/placing telephone calls, filing, and note taking. The employee may be exposed to the following environmental factor- working closely with others. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Occupational Health Clinic policy. There will be intermittent exposure to blood and body fluids.
