The incumbent serves as the Social Work Program Coordinator assigned to the Rehabilitation and Extended Care (REC). The incumbent is responsible for the professional and administrative management of the Veteran Directed Care (VDC) Program in the St. Cloud VAMC.

Administrative - Incumbent provides administrative, technical, budgetary monitoring and oversight necessary for accomplishing the coordination and management functions for the VDC program.
The incumbent performs a wide range of services and provides technical assistance to State, local, and private agencies. Technical assistance provided to monitor activities and program reviews, and the development and analysis of information. In addition, the incumbent will use sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs work related to the review, monitoring, analysis, planning and assessment of each program under his/her authority as well as quality improvement aspects. Responsible for key project activities related to the planning, development, and implementation of the project component.
Program Coordination - As an advanced practitioner, incumbent serves as the Social Work Coordinator of the VDC program. Incumbent duties include all administrative oversight and clinical responsibility for all programs assigned. The incumbent is responsible for weekly, monthly, and quarterly meetings and reports respective to each assigned program. The incumbent is also responsible for program reports to VISN and VACO for the coordinated programs. Seeks guidance from VACO program office as needed. Develops and implements information systems to track service activities including visits, referrals, patient demographics, and patient satisfaction.
Duties include ensuring Medical Center Memorandum compliance with the VDC program. As the Coordinator, incumbent is responsible for scheduling, organizing and inspecting of community provider facilities of each program respectively and completion
of appropriate required reports and follow-up to ensure program operation.
The incumbent is responsible for negotiating, facilitating and completing basic ordering agreements/contracts for the VDC. Incumbent is responsible for the development, coordination, selection and evaluation of VDC in the St. Cloud catchment area; implementation and application of VA guidelines to ensure standards maintained promote health, safety, welfare, and satisfaction of the Veteran receiving community services; coordination of annual inspections and appropriate oversight to ensure compliance with all VA guidelines; assurance of optimal qualitative/quantitative clinical program services via interdisciplinary collaboration, program development, and effective utilization of VA/community resources to provide appropriate services and patient advocacy.
Attending and participating in all required clinical and administrative meetings.
Assessing for signs and symptoms of abuse, neglect and exploitation. The incumbent will report suspicion of abuse, neglect and exploitation when indicated according to VA policy.
Conducting practice in accordance with the VA and discipline specific ethical require.
Collaborating in performance improvement processes and comply with performance measures as required by the VA for specific population/program.
Maintain accurate, timely, and complete clinical records in accordance with professional practice and VA administration guidelines.
Performs other duties as assigned.
Work Schedule: Monday-Friday 7:30am-4pm

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website.
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part.
Grade Determinations: GS-12
Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits).
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs(a document with written demonstration of KSAs must be included with application, see required documents section).
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
