The incumbent will be assigned to the Nursing Department: ICU/Stepdown Nursing Unit. This position is located within the VA Central California Health Care System, Fresno, VACCHCS. The Medical Instrument Technician (MIT) at this level is responsible for performance of all tests within the scope of certification including electrocardiographs and continuous monitoring of cardiac rhythms of ICU and telemetry patients throughout the facility.

Duties

Major duties, include but are not limited to the following: The technician will work with all cardiology staff to efficiently process Holter monitor's or other testing within scope of technicians practice. Thorough knowledge of procedures and policies regarding all areas of responsibilities including but not limited to completing EKG's, Holter monitoring, treadmill testing and other studies within their competencies is necessary. The technician will use the application of these processes to complete all required duties and during course of duty assess priorities to best meet veteran's health care needs. The technician is knowledgeable and independently functions in the use of the monitoring equipment as well as the Communication Software System within V ACCHCS in collaboration with the unit RN or cardiology provider. Information is relayed quickly via in house wireless network, telephones and verbally for notification of emergent alarms and potentially emergent situations. The technician will report rhythm changes and arrythmias to the assigned RN or cardiology provider caring for the patient as outlined in the Telemetry Policy. This requires experience in operating and supervising the operation of specialized monitoring equipment related to the responsibilities of the position, which includes calling a Code Blue if necessary. Reviews previous shift's rhythm strips for shift history and reviews patient information on CPRS. Upon admission, reviews telemetry orders and parameters in order to prevent any delay in placing a patient on ordered telemetry and to ensure accuracy of ordered parameters for monitoring. The technician completes shift to shift report between technicians regarding patient's status for monitoring and brief history of shift events related to telemetry monitoring and vital sign and updates RN's, charge nurses, nursing supervisors and MD's upon request. The purpose of the work is to provide completion of all required cardiac monitoring and testing as ordered. The work contributes to a base of standard nursing or physician care upon which further care may be planned and/or provided by cardiology or nursing staff. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Physical Requirements. See VA Directive and Handbook 5019, Employee Occupational Health Services. This position requires to undergo a pre-employment medical examination/physical, TB test, and other medical related requirements. English Language Proficiency. Medical Instrument Technicians must be proficient in spoken and written English in accordance with chapter 2, section D, paragraph 5a, this part. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-7: (a) Experience. At least 1 year of experience comparable to the next lower grade level which demonstrates the knowledge, skills, abilities, and other characteristics related to the duties of the positions to be filled. This would be experience which provided knowledge of the equipment, standard tests and procedures, and typical readings including arrhythmias and abnormalities. In addition, the candidate must demonstrate the following KSAs:(b) Demonstrated Knowledge, Skills, and Abilities 1. Knowledge of typical patient reactions and signs of distress including the ability to recognize, report and treat potentially lethal arrhythmias.2. Knowledge of common equipment settings and standardized procedures plus knowledge of common errors and corrective measures.3. Ability to modify procedures/positions to obtain the correct results with patients with complicating conditions such as amputations, Parkinson's disease, structural defects, and scar tissue.4. Ability to act as a mentor or preceptor to lower graded technicians.5. Ability to conduct in-service training on the EKG equipment and related instrumentation. **NOTE: MUST REFLECT IN RESUME AND EXPERIENCE IS ONLY CREDIBLE IF DIRECTLY RELATED TO POSITION/SPECIALTY.
