The Facility Program Manager for Formulary Management and Pharmacoeconomics is responsible for facility's pharmacoeconomic program including development, implementation, monitoring and reporting. Programs and program activities shall focus on: pharmaceutical formulary selection, formulary implementation, medication conversions, drug utilization evaluations, medication therapy pathway development and assessment, patient health outcomes, medication education and information communication.

Duties

Assigned Duties and Responsibilities include, but not limited to: Formulary Management and Pharmacoeconomics: Continually identifies and implements strategies to optimize the cost-effective use of medications in the facility. This includes, but not limited to, the implementation of VA clinical practice guidelines, tablet splitting endeavors, procurement, and drug availability. Designs and actively implements conversion strategies due to cost-savings opportunities, evidence updates, or manufacturer backorder and discontinuations. Interfaces with internal and external customers to identify, develop, and implement appropriate cost-containment monitoring opportunities and outcomes research activities. Coordinates the implementation of formulary management, protocols, decision analysis tools, and outcome instruments. Monitors distribution center cost reports to evaluate high-cost and high-utilization medications for potential savings opportunities. Manages and provides oversight for the electronic formulary. Critically reviews non-formulary drug requests for appropriateness and provides feedback to providers. Serves as a resource to Clinical Pharmacy Specialists and Clinical Pharmacists in the approval of questionable requests. Track and trend non-formulary usage by drug, service, and provider to identify target areas of inappropriate usage. Serves as liaison for the Centralized Mail Outpatient Pharmacy (CMOP) program and monitors communications for all logistical issues and potential cost-savings/cost avoidance opportunities. Effectively collaborates with procurement technicians to ensure optimal use of resources and communication with staff regarding changes to drug availability. Program Management: Manages all aspects of pharmacoeconomics to include policy development, implementation and review, quality resources, and specialty functions unique to the program to optimize cost-effectiveness and patient outcomes. Continually reviews economic impact and compliance with pharmacoeconomics activities and provides input to the Pharmacy Management Team. Responsible for national contract compliance, to include tracking and trending usage and providing recommendations and support for implementation. Validates and monitors local performance measure reports related to formulary management and pharmacoeconomic activities. Designs, implements, reports and monitors strategies to meet performance measures as outlined by the VHA, Chief, Pharmacy Service (or designee) and VISN Pharmacy Executive. Provides ongoing input and timely feedback to Chief, Pharmacy Service (or designee) in developing annual budget for Pharmacy Service. Assists in the growth and development of the Pharmacy Service in relation to formulary management and quality assurance. Assists with the routine development of the P&T Committee minutes and the distribution of medical staff informational updates. Plans, implements, and monitors Pharmacy quality assurance activities related to the program. Clinical Practice: Is actively involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Works collaboratively with clinic providers to provide pharmaceutical care to selected patients as outlined by best evidence and clinical practice guidelines. Clinical activities include histories, evaluation, outlining and executing, and monitoring pharmacotherapeutic treatment plans utilizing the most effective, least toxic and most economical medication treatments. Education: In conjunction with the pharmacy managers identifies and plans for continuing education and staff development needs. Maintains competency in the practice of pharmacy, especially in formulary management, outcomes, and information technology. Provides necessary resources for effective pharmacy staff participation in formulary management programs. Provides drug information to other pharmacy staff and healthcare professionals. Administrative: Is responsible for the daily operational status of the various aspects of the pharmacy programs. The incumbent shall keep supervisors and other pharmacy staff members informed of administrative problems or episodes of clinical pertinence which may impact the pharmacy service effectiveness, or which may enhance staff performance. Helps ensure that all aspects of medication accountability, medication security, and timeliness of service are in accordance with local medical center policies and other applicable VHA, FDA, DEA, State and Federal regulations. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English language proficiency in both written and spoken English. Physical requirement: pre-employment physical required. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-13 (a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. (b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
