This position is located at the James A. Haley Veterans' Hospital in Tampa, FL. JAHVAH is a Level I tertiary medical center with general medicine, surgical, psychiatric, rehabilitation, spinal cord injury, nursing home care beds and one of four Polytrauma treatment facilities in the VA. It also encompasses one outpatient clinic and three community-based outpatient clinics. The hospital is a teaching hospital affiliated with the University of South Florida School of Medicine.
Duties
Incumbent will be working with the Geriatrics Outpatient Team to complete the psychosocial portion of the Geriatric Evaluation (GE).
Duties and skills needed to be successful in this position include, but are not limited to:
Perform psychosocial assessments while also working with an interdisciplinary team to address the complex medical needs of the aging Veteran population
Posses the knowledge and ability to independently implement treatment modalities, provide brief counseling or psychotherapy for individuals, families, and groups
Ability to incorporate complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice is highly preferred
Ability to determine priority for services and provide specialized treatment services, based on the complex and varied needs of localized outpatient practice
Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team
Posses the skills to offer psychosocial interventions and measures outcomes to interventions
Independently identifies high-risk patients and provides case management services
Advanced knowledge of community resources as well as VA programs that will support the psychosocial and financial needs of veterans living at home
Incumbent is able to provide the customer with consistent information according to established policies and procedures
Provide crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members
Knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology
Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills
Independently provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan
Ability to evaluate incumbents own practice through professional peer review, case conferences and other organizational means. Incumbent is able to communicate effectively, both orally and in writing, with people from varied backgrounds
Incumbent protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy, and VHA policy
Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion. Incumbent follows applicable regulations and instructions regarding access to computerized files, release of access codes, etc
Work Schedule: Monday-Friday 7:30am- 4:00pm
Telework: : Available (AD-hoc) This position is on-site but may be approved for telework.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Note: A doctoral degree in Social Work may NOT be substituted for the master's degree in Social Work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensed or certified by a state to independently practice social work at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.
Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for GS-11. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.
Experience: Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non VA experience) and licensure or certification in a state at the independent practice level.
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs:
a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
(e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
Preferred Experience: Experience working in the community, using telehealth devices, excellent time management skills and use to work independently.

Physical Requirements: The work is community based and standing and walking are regular daily activities. The work requires that the incumbent drive on a regular basis as a significant amount of his/her time will be spent in the community meeting with Veterans, landlords, and other community agency representatives. A valid driver's license and ability to drive a government vehicle is required.
