The Medical Instrument Technician (Vascular) works for the Vascular/Diagnostic Lab has the responsibility for the performance of vascular and diagnostic ultrasound procedures in the Vascular/Diagnostic lab as well as special projects. Incumbent works closely and assist with the technologists, nurses, clinicians, and clerical staff to assure acquisition of optimum quality control, efficient coordination of activities, high quality diagnostic/therapeutic exam results, and patient satisfaction.
Duties
The Medical Instrument Technician (Vascular) is expected to perform all complex non-invasive vascular testing for diagnostic purposes. These procedures are performed with a variety of testing equipment and modalities to include, but not limited to, B mode scanning, duplex imaging, Doppler spectral analysis, Color Doppler, photoplethysmography, and segment blood pressures. The incumbent must be able to deviate from normal technique when necessary. Duties include but not limited to:
Performs in a hospital setting (inpatient and outpatient) all ultrasounds including the most complex and highly specialized. Performance of the exam requires proper alignment and position of the patient. Sets up and adjusts all equipment and manually maneuvers the transducer to perform the exam. Responsible for obtaining pertinent diagnostic exams through knowledge of both normal and abnormal human anatomy. Shows capability of recognizing indications for further studies beyond established and standardized procedures. Gives preliminary findings of the above-mentioned studies subjects to final approval of the supervising physician.
Receives and identifies patients, explains methods of procedures, assists movement of patients to and from wheelchairs, carts and tables, positions the patient, and performs exams requested.
Coordinates and performs all exam scheduling. Evaluates the nature of critical and emergency procedures and rearranges priorities to accommodate them. Maintains records of patients treated, exams performed, scans taken, etc. Independently sets up inpatient appointments, performs the studies, and records the results.
Assists surgeons and interventional radiologists in the performance of vascular lab procedures. These procedures require monitoring patient and positioning/manipulation of patients to achieve optimal procedure performance.
Works with the interventional radiologists and surgeons as a member of the vascular lab team. Assists and anticipates the needs of these physicians and the performance of the exam. Must be continuously on the alert regarding the patient's condition, and status of equipment and accessories. Assists the radiologists and surgeons with the performance of advanced therapeutic and diagnostic procedures. These procedures require an added level of expertise from the technologist to place diagnostic probes in the performance of ultrasound guided studies.
Provides vascular sonography services in more complex and non-standard cases.
Ensures that adequate quantities of lab supplies are available in designated work area to meet workload demands. Assures that lab procedure rooms are always stocked with necessary supplies with special attention to emergent equipment and supplies.
Assists supervisor in new product evaluation. Recommends methods and procedures to reduce costs and/or increase quality or productivity.
Monitors work performance by contractors and in-house improvements, repairs, upgrades, etc. Assist supervisor in developing specifications for new equipment requests.
Evaluate new products and equipment and make recommendations concerning developments which could improve operations.
Other duties as assigned.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education. There are no specific educational requirements for this occupation. Education may be substituted for experience only at the GS-4 and GS-5 levels. See the grade requirements part of this standard for information regarding educational substitutions.
Licensure or Certification. Licensure or Certification is not required for this occupation; however, it is strongly desirable at GS-6 or above as evidence of possession of the essential knowledge, skills, and abilities. For certain functional areas at the higher levels or supervisory assignments, specific certifications appropriate to the specialty are indicated in this standard under "Titles and Certification/Registrations".
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-9, Medical Instrument Technician
In addition to the core competencies defined at the GS-8 full performance level, positions at this grade may be lead, supervisory, or non-supervisory with specialized advanced knowledge and assignments.
Experience. At least 1 year of experience comparable to the next lower grade level, which is directly related to the duties of the position to be filled and must fully meet the KSAs at that level. This would be experience which demonstrates the knowledge, skills, and abilities to fully perform all vascular ultrasound tests and examinations and to function as a technical director of the vascular laboratory and provide oversight and/or guidance to other technicians. In addition, the candidate must demonstrate the following technical KSAs and the potential to acquire the assignment specific KSAs designated by an asterisk (*): Knowledge, Skills, and Abilities (KSAs): In addition to meeting the experience requirements for this grade level, the candidate must fully demonstrate the following KSAs:
Demonstrated Knowledge, Skills, and Abilities. KSAs 1, 2, and 3 are appropriate for all positions at this level. KSAs 4 and 5 are to be used in accordance with their appropriateness for the specific assignment.
1. Knowledge of the more complex examination and treatment procedures and techniques in order to provide training and supervision in these procedures, to evaluate and initiate performance improvement projects, and to write policies, procedures, and protocols, that pertain to vascular ultrasonography.
*2. Knowledge of JCAHO and other regulatory requirements.
3. Ability to communicate effectively and professionally with employees at varying grade levels.
*4. Ability to manage and supervise employees.
*5. Ability to provide, or provide for, staff development and training.
Certification. Registration by Cardiovascular Credentialing International (CCI) as a Registered Vascular Sonographer (RVS) or by the American Registry of Diagnostic Medical Sonographers (ARDMS) as a Registered Vascular Technologist (RVT) is highly desirable. Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) certifications are also desirable.

The full performance level of this vacancy is GS-9.
