This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. VA Careers - Pharmacy: vacareers.va.gov/careers/pharmacy-jobs/ Primary responsibility of Western Market (WM) Clinical Pharmacy Specialist (Clinical Pharmacy Practitioner - CPP) is the provision of evidence-based renal focused pharmaceutical care services through both direct/indirect interaction with patients and providers. Pharmacist promotes and manages drug therapy based on current clinical knowledge that is consistent with policies and guidelines established at the national, VISN, and WM levels. This pharmacist while working independently through their SOP, will collaborate with renal providers and other team members to address complicated patient management issues. This position plans and provides renal focused clinical pharmacy care to patients located throughout the sites within the Western Market (Pittsburgh, Butler, Erie, and Altoona VA facilities). CPP works with other pharmacists and practitioners (both VA and NON-VA) to provide comprehensive clinical and educational services within the WM. The primary function is managing therapy of patients with varying levels of renal function (including dialysis and post-transplant patients) and associated chronic medical conditions to achieve therapeutic goals and improving success rate on critical WM performance measures. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. OR Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e. Puerto Rico), or the District of Columbia May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Physical Requirements. See VA Directive and Handbook 5019. Pre-employment physical required. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d) May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations GS-13 Pharmacist (Clinical Specialist): Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Such as but not limited to: reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for year basis. The pharmacy residency program must be accredited by the American Society of Health System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable. In addition to the above experience requirement, candidates must also demonstrate the below knowledge, skills, and abilities (KSAs): 1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. 2. Expert understanding of regulatory and quality standards for their program area. 3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. 4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. 5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Preferred PGY-1 Pharmacy Practice, PGY-2 Ambulatory Care, Internal Medicine, Renal, or equivalent experience preferred. Physical Requirements: Incumbent must be physically and mentally able to perform efficiently the duties of the position. The position requires prolonged ability to work in a sitting desk position. The incumbent must have a current, valid driver's license and must be able to drive in all forms of inclement weather conditions. Work is remotely based to cover all four WM sites but if need arises may be performed in person in patient care areas, where there is potential for exposure to communicable disease. Home based work areas should be adequately heated and ventilated, and incumbent should have access to facilities. Some additional travel may be required to attend meetings at other VA facilities. This position operates in a work environment that is subject to frequent, abrupt, and unexpected changes in work assignments due to shifting demands, priorities, and deadlines, which require the employee to constantly adjust operations under the pressure of continuously changing and unpredictable conditions. This position functions in an operation in which work processes and procedures vary constantly because of the impact of changing technology.
