This position will support the Federal Government's response efforts to COVID-19.
This position is located in the White Earth Health Center, Ogema, MN. The purpose of the position is to provide professional nursing care management services to patients in an outpatient clinic setting. The incumbent of this position will report to the Director of Nursing or designee.
Duties
Provides comprehensive nursing care and care management services to patients based on a nursing history/assessment, facility accepted clinical standing orders, policies/procedures and the physician's medical plan of care. Participates as a crucial member and leads the care team.
Utilizes the nursing process in identifying individual patient problems and in developing, implementing and evaluating patient care plans.
Communicates the patient needs and recommended treatment to care team.
Triages patients who call or walk-in and directs them appropriately.
Provides patient education and self-management support by making weekly/monthly calls to encourage patient to meet goals.
Other related duties as assigned.

Education
A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education (external link), Council on Accreditation of Nurse Anesthesia Educational Programs (external link), Accreditation Commission for Midwifery Education (external link), or an accrediting body recognized by the U.S. Department of Education (external link) at the time the degree was obtained.
Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools.
Areas of specialties for positions include, but are not limited to, Acute Care, Anesthesia, Community Health, Family, Geriatric, Home Health, Midwifery, Neonatal, Oncology, Pediatric, Psychiatric, Allergy and Immunology, Women's Health, Cardiovascular, Dermatology, Occupational Health, Orthopedic, Respiratory, or other specialties identified for the position.
Licensure
For all grade levels and positions, applicants must have passed the National Council Licensure Examination (external link). In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills, and abilities to successfully perform the duties of the position. At the GS-09 and above grade level, many positions may require experience in a specialty area of nursing.
Basic Requirements for the GS-10: At least one full year of professional nursing experience (equivalent to the GS-9 grade level).
Examples of Experience for the GS-10 Grade Level: Organize patient care and treatments by incorporating all members in the care team. Improve patient care outcomes. Assess the patient's physical, emotional, spiritual, and social-cultural needs. Promote self-management of chronic disease through helping patients to identify barriers to making healthful choices. Help patient identify barriers to adhere to the prescribed regimen, providing coaching and guidance on positive health behaviors and providing chronic disease management. Assess, plan, implement and evaluate the care of patients with medical, surgical, pediatric, newborn, obstetric, ambulatory and emergency conditions in the primary care clinic. Recognize adverse signs and symptoms and react promptly in emergency situation. Provide chart review to identify patients at risk for increased morbidity and mortality and recommended interventions. Provide patient and family education to assist in understanding and accept implications of care, diagnosis and treatment.
Time In Grade
Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
