The Care Coordinator (CC) for Home Telehealth (HT) is involved with the ongoing assessment, monitoring and case management of Veterans in their residential environment (or their environment of choice) and provides appropriate information to providers and the healthcare system to enable 'just in time' care. Care Coordinators serves as case managers to utilize Home Telehealth (HT) technology in their practice of case management.

Duties

Care Coordinators serves as case managers to utilize Home Telehealth (HT) technology in their practice of case management. Duties include, but are not limited to: Responsible for the development, implementation and evaluation of all Home Telehealth programs. In addition to care coordination, the Home Telehealth nurse will perform administrative duties and are encouraged to serve as local, VISN or national contacts. Makes staffing assignments for patient coverage in the event of staff absenteeism, ensures staff complete their annual competencies and quarterly peer review assignments. Responsible for ensuring continuity of care during emergencies and developing a performance improvement culture. Serve as point of contact for the Home Telehealth Conditions of Participation (COP), addressing compliance, deficiencies, ensuring Home Telehealth meets all identified targets. Responsible for Home Telehealth presence through promotion and awareness relating activities, collaborations and outreach in local settings. The Home Telehealth Coordinator is also responsible for ensuring new staff are oriented to all aspects of the HT program including business, clinical and technical program processes. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Recent Home Telehealth experience, MS office experience, Chronic Care Management experience. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Physical demands include repetitive body movements including the following: frequent reaching, standing, walking, and lifting of equipment and/or patients to provide patient care. This position requires extension of the hand(s) and arm(s) in any direction including above or below shoulder level, during the provision of patient care, including retrieval of needed equipment, supplies and to make contact with the patient. This position requires the maintenance of an upright position to perform duties associated with patient care. This position is typically non-sedentary, and frequently involves being in the standing position. This position requires moving about on feet to accomplish tasks, such as assisting with patient ambulation and moving one patient area to the next. This position requires the raising of objects from a lower position to a higher position or the movement of objects horizontally position to position. Lifting may be done to move a patient from one location to the next, i.e. - from the exam table to the chair, and in the movement of equipment or supplies. This position requires exerting up to 50 pounds of force occasionally and/or up to 25 pounds of force frequently to move objects. This position requires writing or otherwise working with fingers and/or hands, including one or both hands frequently. This is done when taking a patient's pulse, utilizing a stethoscope, administering medications, entering information into the computer, etc. This position requires substantial movements (motion) of the wrists, hands, and/or fingers, as is done when entering patient notes into the computer. This position requires the expression of ideas by means of the spoken word, including the provision of education and instruction to patients and the sharing of information with co-workers. This position requires the perception of sounds at normal range, including the reception of oral communication shared at interdisciplinary team meetings and oral requests from patients. This position requires the ability to see near and far, requires depth perception and the ability to distinguish between basic colors and shades of colors when reading doctor's order, visualizing hue of a patient's skin, read measurements such as thermometers, IV pumps, computer monitors, etc. Specific vision requirements are detailed below: Near vision correctable at 13" to 16" to Jaeger 1 to 4 Far vision correctable in one eye to 20/20 and to 20/40 in the other Far vision correctable in one eye to 20/50 and to 20/200 in the other Specific visual requirement of depth perception.
