The Physician Assistant, in collaboration with the interdisciplinary team, manages the care of outpatients as a Primary Care Provider as part of a Patient Aligned Care Team.
Duties
The Physician Assistant for the Home Based Primary Care Program works as a member of the multidisciplinary Patient Aligned Care Team and provides care to a panel of approximately 900 patients. The Primary Care PA assists with the total care of the patient's medical needs by performing history and physical examinations, promoting preventative health measures, managing acute and chronic conditions, ordering appropriate lab tests, evaluating those tests and acting within guidelines laid down by the supervising physician to ensure quality care of the patient.
Assists in preparation of the patient
Responsible for maintaining medical record entries which reflect the patients healthcare
Provide services as directed to meet the needs of the patient population being served by the medical center.
Duties may include backup coverage to areas under the supervision of Section Chief Primary Care
Use effective decision making and leadership in the application of the care of the patient
Maintains clinic schedule and informs proper parties of same.
Guides patients and families in obtaining the best service and information from internal/external sources

Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~
Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~
Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia.
Preferred Experience: 5 years of Geriatric or Internal Medicine experienceGrade Determinations: In addition to the basic qualifications the following criteria must be met in determining the grade assignment of candidates.Grade 1-Entry Level with knowledge, skills, and abilities that present the minimum skill set expected of an Entry Level PA.Grade 2-Intermediate grade level and has had progressively increasing responsibility and experiences.
Two years of successful practice as a PA OR Successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency. OR A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education.
Grade 3-Full Performance Level and lead and organize patient care independently, practice within multi-disciplinary teams, and coordinate all aspects of patient care across the health care system.
Three years of successful practice as a PA OR successful completion of a PA post-graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, U.S. Armed Forces, or another accrediting agency and on year of OR A doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education and on year of professional practice as a PA.
