**This location is primarily located at Lecanto outpatient clinic. Incumbent will be driving in surrounding area.** Incumbent assigned as the Home-Based Primary Care Social Worker assesses the needs of high-risk patients and provides care management services within the interdisciplinary team. The incumbent works with an interdisciplinary team and provides consultation services to the team and other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems.

Duties

Duties include but are not limited to: Provides administrative oversight of program and administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, inpatient days, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. Independently identifies high-risk patients and provides case management services. Incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to optimize the veteran's functional status and safely maintain the veteran in his/her home. The assessment will include a home visit. It is the responsibility of the social worker, working within the interdisciplinary treatment team to develop a treatment plan with the veteran based on this assessment. Attends weekly interdisciplinary care planning meetings. Ongoing assessments and updating treatment plans are done when necessary or on a quarterly basis given the changing needs as health status changes. Social workers will address the unique needs of homebound, frail, disabling/chronic and/or dying veteran. Incumbent can coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Independently implements treatment modalities, provide counseling or psychotherapy for individuals, families and groups. Makes rapid assessments and developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long-term placements. Social worker has access to multiple resource directories in addition to previously developed resources to meet the demands of a crisis. In the event of a medical or psychiatric emergency, staff will follow the protocol of the HBPC Program. Position will be traveling/driving to Veterans Homes and providing community services. The primary age of patients treated and cared for are normally at the geriatric level, i.e. 60 years of age or older, although occasionally there may be younger patients between the ages of 18-60 years of age that require care. The position requires the incumbent to possess or develop an understanding of the particular needs of these types of patients. Sensitivity to the special needs of all patients in respect to age, developmental requirements, and culturally related factors must be consistently achieved. Qualifications

Applicants must meet the requirements by the closing date 01/17/2024 of this announcement. Basic Requirements: United States Citizenship: Must be US Citizen. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker Program Coordinator candidates must have the following: Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Program Coordinator social workers have experience that demonstrates possession of advanced practice skills and judgment. Social workers are experts in their specialized area of practice. They may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Training or additional certs must be submitted with application. AND Licensure/Certification: At this grade level, social workers must be licensed or certified by a state at the advanced independent practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Advanced License must be submitted with application. References: VA Regulations, specifically VA Handbook 5005/120, Part II, Appendix G39, Social Worker Qualification Standard, dated 9/10/19. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Incumbent must have a valid driver's license and pass Occupational Health screening as position will require driving a government vehicle. Incumbent must be able to climb stairs and walk short distances. Incumbent must be able to lift 25 pounds.
